日本語
徳島大学病院に搬送された内因性くも膜下出血（SAH）患者を対象に、Case-Crossover Designを用いて、体感気温の急激な低下および大気汚染物質（PM2.5、NO2、Ox）への曝露がSAH発症リスクに与える因果関係とその遅延効果を解明する。さらに、動脈瘤のサイズ、部位、形態（blebの有無）、およびUCAS Japanリスク層別化スコアに基づき、気象・大気環境環境変動に対する動脈瘤の感受性の違い（交互作用）を明らかにし、高リスク患者に対する個別化予防医療の基盤を構築に貢献することを目的とする。

The objective of this study is to elucidate, through a case-crossover design, the causal relationship and lag effects between rapid decreases in apparent temperature, exposure to ambient air pollutants (PM2.5, NO2, and Ox), and the onset of spontaneous subarachnoid hemorrhage (SAH) in patients admitted to Tokushima University Hospital. In addition, we aim to determine whether susceptibility to meteorological and air environmental fluctuations differs according to aneurysm characteristics, including size, location, morphology (presence of blebs), and UCAS Japan risk stratification score, through interaction analyses. The ultimate goal is to provide evidence supporting the development of personalized preventive medicine for individuals at high risk of aneurysm rupture.

2020年1月1日から2025年9月30日までの期間に、徳島大学病院SCUデータベースに登録され、同院へ緊急搬送・入院となった内因性SAH患者（目標症例数：200例）。発症日および発症時刻、居住地（あるいは発症地）が明確に診療録から同定できる症例を対象とする。

This study will include patients with spontaneous subarachnoid hemorrhage (SAH) registered in the Tokushima University Hospital SCU database who were emergently transported to and admitted to the hospital between January 1, 2020, and September 30, 2025 (target sample size: 200). Cases with clearly identifiable onset date and time and residential address or location of onset in the medical records will be included.

除外基準/Key exclusion criteria

1) 外傷性SAH症例
2) 発症日時または居住地情報が不明で、気象・環境データの紐付けが不可能な症例
3) 本研究への情報利用について、オプトアウト（情報公開文書等）を通じて研究対象者または代諾者から明示的に参加拒否の申し入れがあった症例

1. Patients with traumatic subarachnoid hemorrhage (SAH).

2. Patients for whom the date and time of onset and/or residential location information are unavailable, making linkage with meteorological and environmental data impossible.

3. Patients for whom an explicit refusal to participate was expressed by the patient or their legally authorized representative through the opt-out process (e.g., information disclosure document) regarding the use of their information in this study.

本研究は、徳島大学病院SCUデータベースを用いた単施設後ろ向き観察研究である。2020年1月1日から2025年9月30日までに徳島大学病院へ緊急搬送・入院となった内因性くも膜下出血患者を対象とする。

研究デザインにはCase-Crossover Designを採用し、各症例を自身の対照として解析する。発症日時および居住地（または発症地）情報と、気象データ（体感気温等）および環境データ（PM2.5、NO2、Ox等）を連結し、発症リスクとの関連ならびに遅延効果（lag effect）を評価する。

さらに、動脈瘤サイズ、部位、形態（blebの有無）およびUCAS Japanリスク層別化スコアに基づく層別解析および交互作用解析を実施し、気象・環境因子に対する動脈瘤の感受性の違いを検討する。

This is a single-center retrospective observational study using the Tokushima University Hospital Stroke Care Unit (SCU) database. Patients with spontaneous subarachnoid hemorrhage (SAH) who were emergently transported to and admitted to Tokushima University Hospital between January 1, 2020, and September 30, 2025, will be included.

A case-crossover design will be employed, with each patient serving as his or her own control. Information on the date and time of onset and place of residence (or location of onset) will be linked with meteorological data (including apparent temperature) and environmental data (including PM2.5, NO2, and Ox) to evaluate their associations with the risk of SAH onset and to assess potential lag effects.

Furthermore, stratified and interaction analyses will be performed based on aneurysm size, location, morphology (including the presence of blebs), and the UCAS Japan risk stratification score to investigate differences in aneurysm susceptibility to meteorological and environmental factors.
