心臓血管外科手術において、人工心肺離脱後の止血管理は手術時間や予後に大きく影響する重要な因子である。フィブリノゲン値および血小板数は止血能を反映する指標であるが、これらが手術時間に与える影響については十分に検討されていない。本研究では、人工心肺離脱直後のフィブリノゲン値および血小板数と手術時間との関連を明らかにすることを目的とする。また、副次的目的として、人工心肺離脱直後のフィブリノゲン値および血小板数と、人工心肺離脱後から手術終了までの総出血量との関連についても検討する。

This study aims to evaluate whether fibrinogen levels and platelet counts measured immediately after separation from cardiopulmonary bypass are associated with time to surgical completion. The association between these hemostatic parameters and post-bypass blood loss will also be investigated.

1. 胸骨正中切開での人工心肺を用いた心臓血管外科手術
2. 人工心肺離脱後、30分以内にフィブリノゲン値と血小板数が同時に測定されている患者

Adult patients undergoing cardiovascular surgery with cardiopulmonary bypass through a median sternotomy.
Patients in whom both fibrinogen level and platelet count were measured within 30 minutes after separation from cardiopulmonary bypass.

除外基準/Key exclusion criteria

日本語
1. 手術中に再度人工心肺開始（2nd pump run）となった患者
2. 主要解析項目（離脱直後のフィブリノゲン値、血小板数）が欠測している症例

1. Patients who required reinstitution of cardiopulmonary bypass during the same surgical procedure (second pump run).
2. Patients with missing data for the primary explanatory variables, including fibrinogen level or platelet count measured immediately after separation from cardiopulmonary bypass.

本研究は、診療録情報を用いた単施設後ろ向き観察コホート研究である。胸骨正中切開下に人工心肺を用いた心臓血管外科手術を受けた成人患者を対象とし、人工心肺離脱直後のフィブリノゲン値および血小板数が、人工心肺離脱後から手術終了までの時間ならびに総出血量に及ぼす影響を検討する。解析には一般化線形混合効果モデルを用い、術者間のばらつきを考慮して評価を行う。

This is a single-center retrospective observational cohort study using routinely collected clinical data. The study will evaluate the associations of fibrinogen level and platelet count measured immediately after separation from cardiopulmonary bypass with time to surgical completion and post-bypass blood loss in adult patients undergoing cardiovascular surgery with cardiopulmonary bypass.
