冠動脈CTは従来冠動脈疾患リスクが低-中リスクの患者におけるスクリーニング検査として推奨され、冠動脈の解剖学的評価に優れ陰性的中率が高いことが知られている。プラーク形態や石灰化の評価は以前から行われていたが、近年はさらに冠動脈や心筋の質的評価を行うことができるようになり、冠動脈周囲脂肪織濃度減衰指数による炎症の評価や細胞外容積分画により線維化や浮腫の評価ができるようになった。一方でこれらの指標が実際の患者プロファイルや心エコー所見、冠動脈カテーテル所見、患者予後とどう関連するかについては十分に検証されていないため、その臨床的意義を探索する。

Coronary CT angiography is traditionally recommended as a screening test for patients with low-intermediate risk of coronary artery disease. It is renowned for its ability to provide an excellent anatomical assessment of the coronary arteries and its high negative predictive value. While assessing plaque morphology and calcification has been possible for some time, it has only recently become feasible to conduct qualitative evaluations of the coronary arteries and myocardium. This enables the evaluation of inflammation through the pericoronary fat attenuation index (FAI), as well as the assessment of fibrosis and oedema using the extracellular volume fraction (ECV). However, as the relationship between these indicators and patient profiles, echocardiographic findings, coronary catheter findings and prognosis has not yet been fully verified, we will investigate their clinical significance.
