選択基準/Key inclusion criteria

日本語
1) 切除不能悪性肝門部胆管閉塞症例
2) 病理学的検査で悪性疾患の診断が得られた症例
3) 血清総Bil、AST、ALT、ALP、g-GTPのいずれかに異常値を認め、胆管ドレナージを要する症例
　異常値の基準は下記に記す。
　・総Bil >2 mg/dL
　・AST >45 IU/L
　・ALT >45 IU/L
　・ALP >170 IU/L
　・g-GTP >75 IU/L
4) 初回両葉インサイドプラスチックステント留置症例
5) 2カ月以上の予後が望める症例
6) Bismuth type II-IV症例（両葉胆管ドレナージが必要な症例）
7) 18歳以上

Patients with unresectable malignant hilar biliary obstruction
Patients with a pathological diagnosis of malignancy
Patients requiring biliary drainage due to abnormal values in any of the following: serum total bilirubin, AST, ALT, ALP, or g-GTP
The criteria for abnormal values are as follows
Total bilirubin more than 2 mg/dL
AST more than 45 IU/L
ALT more than 45 IU/L
ALP more than 170 IU/L
g-GTP more than 75 IU/L
Patients undergoing initial bilateral inside plastic stent placement
Patients expected to have a prognosis of at least 2 months
Patients with Bismuth type II-IV (requiring bilateral biliary drainage)
Age more than 18 years

除外基準/Key exclusion criteria

日本語
1) 遠位胆管閉塞を伴う症例
2) 術後腸管症例(Billroth-I法を除く)
3) 他臓器に重篤な基礎疾患を持つ症例
4) 胆管挿管(経乳頭的処置)が行えない症例
5) 十二指腸閉塞を伴う症例
6) ECOG PS 4
7) その他、医師が不適と判断した症例

Patients with concomitant distal biliary obstruction
Patients with surgically altered anatomy (excluding Billroth I reconstruction)
Patients with severe comorbidities affecting other organs
Patients in whom biliary cannulation (transpapillary approach) is not feasible
Patients with concomitant duodenal obstruction
ECOG performance status 4
Patients deemed unsuitable by the treating physician
