潰瘍性大腸炎およびクローン病患者において、尿中PGE-MUMの疾患活動性評価および治療予後予測バイオマーカーとしての有用性を検討する。

研究機関の長の許可日～2028年3月31日までに、岡山大学病院および共同研究機関に通院あるいは入院中で、新規に5-ASA製剤、ステロイド（プレドニゾロン最大１日投与量30 mg以上）、分子標的薬、低分子化合物、タクロリムス、免疫調整薬、血液浄化療法による新規治療の開始あるいは治療の変更を行うUCあるいはCD患者を対象に、保険診療内で定期的に測定する血液・便・尿検査と同時に、尿中PGE-MUMも併せて測定を行い、疾患活動性との相関性、血清・便中バイオマーカーとの相関性を評価する。また、疾患活動性の変化と尿中PGE-MUMを含めたバイオマーカーの推移の相関性も検討するため、一般的な診療の範囲内で、定期的に一般的な血液・便検査に加えてPGE-MUM測定のための尿検査も行う。

From the date of approval by the head of the research institution to March 31, 2028, UC or CD patients receiving outpatient or inpatient care at Okayama University Hospital or participating institutions who newly start or change treatment with 5-ASA, corticosteroids, molecular-targeted agents, small-molecule drugs, tacrolimus, immunomodulators, or blood purification therapy will be enrolled. Urinary PGE-MUM will be measured together with routine blood, stool, and urine tests performed in standard clinical practice. The associations of urinary PGE-MUM with disease activity and serum or fecal biomarkers will be evaluated. Longitudinal associations between changes in disease activity and changes in biomarkers, including urinary PGE-MUM, will also be examined.

除外基準/Key exclusion criteria

①診断未確定者
②潰瘍性大腸炎で大腸全摘後の患者
③活動性の悪性腫瘍がある患者
④妊婦・授乳中の患者
⑤本研究に対して同意を得ていない患者
⑥研究責任者が研究対象者として不適当と判断した患者
