日本語
本研究は、稀な食道静脈瘤の一亜型であるパイプライン型食道静脈瘤を対象とした多施設共同の後ろ向きコホート研究である。本研究では、患者背景、血行動態、治療内容などの臨床情報を多施設から収集・解析し、本疾患の臨床的特徴および血行動態を明らかにすることを目的とする。さらに、これらのデータを基にデータベースを構築し、パイプライン型食道静脈瘤に対する適切な治療戦略の確立に資する知見を得ることを目指す。

This multicenter retrospective cohort study aims to clarify the clinical characteristics and hemodynamic features of pipeline esophageal varices, a rare subtype of esophageal varices. Clinical data, including patient background, hemodynamic findings, and treatment modalities, will be collected and analyzed from multiple institutions. In addition, a database will be established based on these data to generate findings that may contribute to the development of appropriate management strategies for pipeline esophageal varices.

（造影CTまたはEVIS画像により評価した食道静脈瘤の最大短径、供血路の種類〔左胃静脈、後胃静脈、短胃静脈〕、供血路の最大短径、脾静脈最大短径、門脈大循環シャントの有無）

Hemodynamic findings at the time of diagnosis of pipeline esophageal varices
(assessed by contrast-enhanced CT and/or EVIS images, including the maximum short diameter of the esophageal varix, type of feeding vessel [left gastric vein, posterior gastric vein, or short gastric vein], maximum short diameter of the feeding vessel, maximum short diameter of the splenic vein, and presence of portosystemic shunts)

副次アウトカム評価項目/Key secondary outcomes

（年齢、性別、BMI、飲酒歴、喫煙歴、食道静脈瘤治療歴、HCC治療歴、生存状況）
２．パイプライン型食道静脈瘤診断時の血液検査所見
（白血球数、ヘモグロビン値、血小板数、総蛋白、アルブミン、AST、ALT、ALP、γGTP、BUN、クレアチニン、PT、PT-INR、D-dimer、FDP、AT-III、アンモニア、抗核抗体、抗ミトコンドリア抗体、抗ミトコンドリアM2抗体、ヒアルロン酸、Ⅳ型コラーゲン4S、M2BPGi）
３．パイプライン型食道静脈瘤診断時の内視鏡所見
（Location、胃静脈瘤の有無、食道静脈瘤との連続性・交通の有無、Form、red color signの有無）
４．パイプライン型食道静脈瘤に対する治療内容
（内視鏡治療、薬物療法、IVR、外科手術の実施内容）
５．全生存期間
（パイプライン型食道静脈瘤診断日から最終生存確認日または死亡日まで）

1. Patient background characteristics and clinical findings at the time of diagnosis of pipeline esophageal varices
(age, sex, BMI, alcohol history, smoking history, previous treatment for esophageal varices, history of HCC treatment, and survival status)
2. Laboratory findings at the time of diagnosis of pipeline esophageal varices
(white blood cell count, hemoglobin, platelet count, total protein, albumin, AST, ALT, ALP, gamma-GTP, BUN, creatinine, PT, PT-INR, D-dimer, FDP, AT-III, ammonia, antinuclear antibody, antimitochondrial antibody, antimitochondrial M2 antibody, hyaluronic acid, type IV collagen 4S, and M2BPGi)
3. Endoscopic findings at the time of diagnosis of pipeline esophageal varices
(location, presence of gastric varices, continuity/communication with esophageal varices, form, and presence of red color signs)
4. Treatment modalities for pipeline esophageal varices
(endoscopic treatment, medical treatment, IVR, and surgical treatment)
5. Overall survival
(from the date of diagnosis of pipeline esophageal varices to the date of last confirmed survival or death)

・2000年1月1日から2025年10月31日までの間に、参加施設においてパイプライン型食道静脈瘤と診断された患者
・通常型食道静脈瘤群については、同期間に山形大学医学部附属病院において内視鏡治療を施行された患者
・年齢制限なし

Patients diagnosed with pipeline esophageal varices at participating institutions between January 1, 2000 and October 31, 2025
For the control group, patients who underwent endoscopic treatment for conventional esophageal varices at Yamagata University Hospital during the same period
No age restriction

本研究は、多施設共同の後ろ向き観察研究であり、研究として新たな介入は行わない。稀な病態であるパイプライン型食道静脈瘤の臨床的特徴および血行動態を明らかにするため、多施設から症例を集積して解析を行う。比較対象として、山形大学医学部附属病院における通常型食道静脈瘤症例をコントロール群として用いる。インフォームド・コンセントはオプトアウト方式により実施する。

This study is a multicenter retrospective observational study, and no additional intervention will be performed as part of the study. To clarify the clinical characteristics and hemodynamic features of pipeline esophageal varices, which is a rare condition, cases will be collected and analyzed from multiple institutions. Conventional esophageal varices cases at Yamagata University Hospital will be used as the control group. Informed consent will be obtained using an opt-out procedure.
