日本語
中小規模運送会社に勤務するトラックドライバーを対象に、客観的な健康診断データ（BMI、血圧、血糖値、脂質値等の心血管代謝リスク因子）と、WEBアンケートで収集した労働負荷・休息条件（運行形態、夜間運転回数、睡眠時間、休日の過ごし方等）の関連性を横断的に検証することを目的とする。特に、夜間運転を含む勤務負荷の程度および休息条件の不足が客観的な心血管代謝リスク指標と有意に関連するか（仮説1）、また不適切な休息条件が主観的な疲労の蓄積だけでなく客観的な心血管代謝リスク指標の上昇に対して独立した寄与要因となるか（仮説2）を検討する。

This study aims to cross-sectionally examine the association between objectively measured cardiometabolic risk factors (CMRFs) from health examination data (BMI, blood pressure, blood glucose, lipid levels, etc.) and work-rest conditions (driving patterns, frequency of night driving, sleep duration, days off activities, etc.) collected via a web-based questionnaire among truck drivers employed at small-to-medium-sized transport companies. Specifically, the study tests whether the degree of work burden including night driving and inadequate rest conditions are significantly associated with objective CMRFs (Hypothesis 1), and whether inadequate rest conditions independently contribute to elevated objective CMRFs beyond subjective fatigue accumulation (Hypothesis 2).

客観的に測定された心血管代謝リスク因子（CMRFs）の有無または程度： ① 体型/肥満指標：BMI（≧25を肥満と定義）および腹囲 ② 血圧：収縮期血圧（SBP）および拡張期血圧（DBP）、高血圧の定義（SBP≧140mmHgまたはDBP≧90mmHg） ③ 脂質代謝指標：総コレステロール、LDLコレステロール、HDLコレステロール、トリグリセリド ④ 糖代謝指標：血糖値およびHbA1c

1. 国際医療福祉大学成田病院予防医学センターで定期健康診断を受診している中小規模運送会社3社に現職のトラックドライバーとして雇用されている者 2. 研究実施期間の直近1年以内に同センターにて定期健康診断を受診しており、心血管代謝リスク指標を含む健康診断データが利用可能である者 3. 本人から文書（電子媒体）による自由意思に基づくインフォームド・コンセントが取得できる者 4. 同意取得時における年齢が20歳以上65歳以下である者 5. 主な業務内容が商用トラックの運転業務である者

Currently employed as a truck driver at one of 3 small-to-medium-sized transport companies that receive periodic health examinations at the International University of Health and Welfare (IUHW) Narita Hospital Preventive Medicine Center 2. Has undergone a periodic health examination at the Center within the past year, with available CMRFs data 3. Able to provide written (electronic) informed consent voluntarily 4. Aged 20-65 years at the time of consent 5. Primary job duty is commercial truck driving

1. 研究参加への同意を撤回した者、または同意が得られない者 2. アンケート回答時点で、長期休業（3か月以上の病気休職や育児休業等）により継続的に運転業務に従事していない者 3. 健康診断データやWEBアンケートの回答から、研究実施に必要な情報が欠損している者 4. 重度の精神疾患、認知機能障害等により、WEBアンケートへの自己回答や研究への参加が困難であると研究責任者が判断した者

本研究は、中小規模運送会社に勤務するトラックドライバーを対象とした横断的関連調査である。既存の健康診断データから得られた客観的な心血管代謝リスク因子（CMRFs：BMI、血圧、脂質、血糖等）と、WEBアンケートで収集した労働負荷・休息条件（運行形態、夜間運転回数、睡眠時間、休日の過ごし方等）の関連性を検証する。統計手法として、多重ロジスティック回帰分析（CMRFs有無を目的変数）、重回帰分析（CMRFs連続値を目的変数）、階層的回帰分析（休息条件の独立寄与の検証）、ノンパラメトリック検定（群間比較）を用いる。年齢、飲酒、喫煙を調整変数とする。解析にはIBM SPSS Statisticsを使用し、有意水準は両側5%未満とする。

This is a cross-sectional association study targeting truck drivers at small-to-medium-sized transport companies. The study examines the association between objectively measured CMRFs (BMI, blood pressure, lipids, blood glucose) from existing health examination data and work-rest conditions (driving patterns, night driving frequency, sleep duration, days off activities) collected via a web-based questionnaire. Statistical methods include multiple logistic regression (CMRFs presence/absence as outcome), multiple linear regression (CMRFs continuous values as outcome), hierarchical regression (testing independent contribution of rest conditions), and nonparametric tests (group comparisons). Age, alcohol consumption, and smoking are adjusted as covariates. IBM SPSS Statistics is used with a two-sided significance level of 5%.
