本研究の目的は、既存のピアサポート機能尺度を基盤として、通所リハビリテーションまたは通所介護を利用する要介護高齢者に適用可能なピアサポート機能尺度を開発することである。まず、専門家によるフォーカスグループ法と内容的妥当性評価により項目を修正し、その後、利用者を対象に表面的妥当性を確認した上で、多機関共同の横断的調査を実施する。得られたデータを用いて因子構造、内的一貫性、再検査信頼性、測定誤差を検討し、さらにSF-8との関連を用いて解釈可能性を検討する。

The purpose of this study is to develop a peer support function scale applicable to older adults requiring long-term care who use day-care rehabilitation or day-care services, based on the existing Peer Support Function Scale. First, the items will be modified through a focus group of experts and content validity assessment. After confirming face validity among service users, a multicenter cross-sectional survey will be conducted. Using the collected data, the factor structure, internal consistency, test-retest reliability, and measurement error of the scale will be examined, and interpretability will be explored through its association with the SF-8.

本研究は、多機関共同の観察研究であり、主たる部分は横断的調査研究である。まず、既存のピアサポート機能尺度について、要介護高齢者支援の実務経験者および質的研究経験者を含む9名の専門家を対象にフォーカスグループ法を実施し、各項目の関連性、包括性、理解可能性を検討して項目修正を行う。次に、修正後尺度について内容的妥当性（CVR、I-CVI、S-CVI/Ave）を評価し、その後、各施設利用者を対象に表面的妥当性を確認する。最終的に作成した尺度案を用いて、多機関共同の横断的調査を実施する。対象者は、研究実施施設において通所リハビリテーションまたは通所介護を利用している65歳以上の要介護高齢者であり、認知症高齢者の日常生活自立度がIまたは問題なし、現在利用中のサービス利用期間が3か月を超え、本人同意が得られた者とする。対象者の募集は、研究対象者と直接的な利害関係のない療法士等が、各施設で選択基準を満たす利用者に直接依頼する方法で行う。募集期間は倫理審査承認日から2028年3月31日までとする。収集項目は、年齢、性別、身長、体重、主病名、精神疾患の有無、障害高齢者の日常生活自立度、世帯、介護保険負担割合、サービス利用年数、Charlson併存疾患指数、歩行自立度、認知症高齢者の日常生活自立度、ピアサポート機能尺度、SF-8である。ピアサポート機能尺度の再現性を確認するため、初回回答から約2週間後に一部対象者へ再回答を求める。

This multicenter observational study primarily uses a cross-sectional design to develop a Peer Support Function Scale for older adults requiring long-term care. First, nine experts in long-term care support and qualitative research will review the existing scale through a focus group to assess relevance, comprehensiveness, and comprehensibility, and to revise items as needed. Second, content validity will be evaluated using the CVR, I-CVI, and S-CVI/Ave. Third, face validity will be examined among users at participating institutions. Finally, a multicenter cross-sectional survey will be conducted using the revised draft scale. Eligible participants are adults aged 65 years or older who require long-term care, use day-care rehabilitation or day-care services, have an independence level in daily living for older adults with dementia of level I or no cognitive problem, have used the service for more than 3 months, and provide written informed consent. Participants will be recruited by therapists or other staff without a direct conflict of interest. Recruitment will continue from ethics approval until March 31, 2028. Variables include age, sex, height, weight, primary diagnosis, mental disorder, independence in daily living, household composition, co-payment rate, years of service use, Charlson Comorbidity Index, walking independence, dementia-related independence level, the peer support scale, and SF-8. To assess test-retest reliability, some participants will complete the scale again about 2 weeks later.
