本研究の目的は、TKA術後の肥厚性瘢痕形成を抑制するため、術後1か月からステロイドテープを開始する超早期介入が、術後3か月から開始する晩期介入と比較して、瘢痕の厚みおよびかゆみNRSをより効果的に改善するかを検証することである。

The objective of this study is to determine whether ultra-early initiation of steroid tape therapy at 1 month after total knee arthroplasty is more effective in reducing hypertrophic scar thickness and improving pruritus NRS scores compared with delayed initiation at 3 months postoperatively.

術後3か月からステロイドテープ貼付を開始する晩期治療。標準的な創管理として、術後3か月以降にステロイドテープを貼付し、瘢痕の厚みやかゆみを経時的に評価する。

選択基準/Key inclusion criteria

選択基準
- 原発性変形性膝関節症に対して初回人工膝関節全置換術（TKA）を施行した患者
- 術後1か月時点で創部が上皮化し、感染徴候を認めない患者
- ステロイドテープ治療を希望し、研究参加に文書同意が得られた患者
- 術後6か月まで経過観察が可能な患者

Key inclusion criteria
1. Patients undergoing primary total knee arthroplasty for knee osteoarthritis.
2. Patients with a fully epithelialized incision and no signs of infection at 1 month postoperatively.
3. Patients who wish to receive steroid tape therapy and provide written informed consent.
4. Patients who are able to complete follow-up assessments up to 6 months after surgery.

除外基準/Key exclusion criteria

除外基準
- 術後1か月時点で創部に感染徴候（発赤、排膿、熱感など）を認める患者
- 創部の上皮化が不十分で、ステロイドテープ貼付が安全に行えない患者
- ステロイド外用剤またはテープ素材（粘着剤を含む）に対するアレルギーを有する患者
- 免疫抑制療法中、または全身性ステロイド治療を受けている患者
- 創傷治癒に影響を及ぼす可能性のある重度の皮膚疾患を有する患者
- 再置換TKA（revision）や感染性関節炎など、原発性TKA以外の症例
- 術後6か月までのフォローアップが困難と判断される患者
- 研究参加に文書同意が得られない患者

Exclusion criteria
- Patients who show any signs of wound infection (e.g., erythema, discharge, warmth) at 1 month postoperatively.
- Patients whose incision is not fully epithelialized and for whom steroid tape application is not considered safe.
- Patients with known allergies to topical steroids or adhesive materials used in the tape.
- Patients receiving immunosuppressive therapy or systemic corticosteroid treatment.
- Patients with severe dermatologic conditions that may affect wound healing.
- Patients undergoing revision TKA or those with infectious arthritis or other non-primary TKA indications.
- Patients who are unlikely to complete follow-up assessments up to 6 months after surgery.
- Patients who are unable or unwilling to provide written informed consent.
