患者が感じる回復（Global Rating Scale）を基準（アンカー）として、回復期病棟でルーチンに取得される機能評価指標の最小重要変化（MIC）を推定する。さらに、入棟から退院までの月次評価の縦断データから回復軌跡クラスを同定し、各クラスの臨床的特徴および治療要因（訓練時間等）との関連を明らかにする。

①脳梗塞または脳出血と診断され、急性期治療後に回復期リハビリテーション病棟へ入棟した者 ②入棟時から退院時までのルーチン評価（本研究で定める評価項目）が取得可能 ③オプトアウトにより研究参加を拒否していない者

本研究は単施設前向き観察コホート（レジストリ）研究である。対象者は2026年5月から2029年3月に当院回復期リハビリテーション病棟へ入棟した脳卒中患者のうち選択基準を満たす連続症例とする。評価は入棟時・退院時（±3日）および入棟中の毎月（±1週間）にルーチンで実施される指標（NIHSS、BRS、FMA上肢/下肢、ARAT、歩行速度、FAC、MMSE、BBS、運動FIM/認知FIM、膝伸展筋力、握力、mRS）を用いる。患者報告型アウトカムとして退院時にGlobal Rating Scale（7段階）を取得し、これをアンカーとして各指標の最小重要変化（MIC）を推定する。さらに月次縦断データを用いて潜在クラス成長分析により回復軌跡クラスを同定し、年齢、性別、発症前mRS、診断名、併存疾患、訓練時間等との関連を検討する。研究は通常診療で得られる情報のみを用い、情報公開文書によるオプトアウト方式で実施する。

This is a single-center prospective observational cohort registry. Consecutive stroke patients admitted to a convalescent rehabilitation ward between [May/2026] and [March/2029] will be included. Routine outcomes are collected at admission, monthly during hospitalization, and at discharge. A 7-point Global Rating Scale at discharge will be used as an anchor to estimate MIC for each outcome. Recovery trajectory classes will be identified using latent class growth analysis, and associations with baseline characteristics and therapy time will be examined. An opt-out approach will be used.
