日本語
ERCPにおける胆管挿管困難時のサルベージ法の一つとして、膵管内にGWを留置した状態で別のGWを用いて胆管挿管を試みる Double guide wire Technique（DGT） がある。DGTは困難胆管挿管例における胆管挿管成功率を改善しうることが報告されているが、その一方で、膵管GW留置や膵管操作の増加によりPEPリスクが上昇する可能性も指摘されている。ただし、DGTのいずれの行程がPEP発症や重症化と関与しているかに関しては十分わかっていないため、今回多施設観察研究でDGTにおけるPEPリスク因子を調査した。

The double-guidewire technique (DGT), in which a second guidewire is used to attempt biliary cannulation while a guidewire is left in the pancreatic duct, is one of the salvage techniques for difficult biliary cannulation during ERCP. Although DGT has been reported to improve biliary cannulation success in difficult cases, it may also increase the risk of post-ERCP pancreatitis (PEP) because of pancreatic guidewire placement and repeated pancreatic manipulation. However, it remains unclear which components of the DGT procedure are associated with the development or severity of PEP. We therefore conducted a multicenter observational study to investigate the risk factors for PEP in patients undergoing DGT.

除外基準/Key exclusion criteria

ⅰ)　ERCP歴のある症例（過去にERCPを企図し胆管挿管不成功であった症例はERCP初回施行例として扱う。）
ⅱ）　膵管病変のみを目的とするERCPの症例
ⅲ） Billroth-I再建を除く再建腸管の症例
ⅳ) 膵切除後の症例
ⅴ) 膵胆管合流異常症の症例
ⅵ） 研究責任医師又は研究分担医師が本研究に不適と判断した症例

[Exclusion Criteria]
i) Patients with a history of ERCP
Cases in which ERCP had been attempted previously but biliary cannulation was unsuccessful will be regarded as first-time ERCP cases.
ii) Patients undergoing ERCP solely for pancreatic duct-related diseases
iii) Patients with surgically altered anatomy, excluding Billroth I reconstruction
iv) Patients who have undergone prior pancreatic resection
v) Patients with pancreaticobiliary maljunction
vi) Patients judged by the principal investigator or subinvestigators to be inappropriate for inclusion in this study
