日本語
筋骨格系の痛みは日本で頻度が高く、医療費の増加と関連している。交通事故後の頸椎・腰椎捻挫などの軽傷は、第三者支払による補償制度下では外来受診の反復や高額な医療費請求につながりやすい。医療利用や請求費用には地域や医療機関間でばらつきがあり、臨床的必要性のみでは十分に説明できないことが報告されている。また、少数の患者や医療機関が総医療費の多くを占める「費用の集中」も知られている。交通事故診療はこれらを検討する上で有用である。
補償制度下では、医療利用は臨床的必要性に加え、制度運用や経済的インセンティブの影響を受ける可能性があるが、軽微な外傷における施設レベルでの費用集中やばらつきの定量的証拠は限られている。さらに、補償医療では痛みや機能障害の評価が給付や手続きと関連し、受療行動や治療経過に影響する可能性がある。補償は症状遷延、治療長期化、医療費増加と関連し、補償制限により請求や症状報告が減少する可能性も示唆されている。心理社会的要因も症状遷延や受療と関連する。
日本の全国データでは請求額は職業、地域、複数受診と関連し、制度設計や医療提供者行動の影響が示唆される。我々の先行研究でも事故重症度は治療期間や受診回数と関連せず、重症度のみでは施設間差を説明できない可能性がある。日本では自賠責制度下で出来高払いが中心で、受診ごとの補償や休業補償が受療行動に影響している可能性がある。

Musculoskeletal pain is common in Japan and is associated with increased healthcare costs. After motor vehicle accidents, minor injuries such as cervical and lumbar sprains often lead to repeated outpatient visits and high claimed costs, particularly under compensation systems financed by third-party payers. Variation in healthcare utilization and claimed costs across regions and providers is not fully explained by clinical need. Cost concentration, in which a small subset of patients or facilities accounts for a large share of total costs, is also recognized.
In compensation systems, healthcare utilization may be influenced by administrative processes and financial incentives as well as clinical need. However, quantitative evidence on facility-level variation and cost concentration for minor injuries remains limited. Compensation has been associated with prolonged symptoms, extended care, and higher costs, while limiting compensation may reduce claims. Psychosocial factors are also related to persistent symptoms and care seeking.
In Japan, claimed costs are associated with occupation, region, and care across multiple institutions. Our previous study found no association between accident severity and treatment duration or visit frequency. Care is mainly reimbursed on a fee-for-service basis under the compulsory automobile liability insurance system, and per-visit compensation and wage-loss benefits may influence care-seeking behavior.

選択基準/Key inclusion criteria

損害保険料率算出機構から提供されるデータで、選択基準は、(1)自動車交通事故(追突、接触、衝突)による傷害、(2)主病名が腰椎捻挫、(3) 自賠責保険の補償による医学的治療が行われた、(5)2014年6月から2019年12月までに自賠責保険を用いた補償が終了した、(6)傷害による損害が40万円以下で後遺障害がない(日本の無過失政府補償では少額の区分)、または傷害による損害が120万円未満で後遺障害(日本の無過失政府補償では軽微な後遺障害)があることとした。

The data provided by the General Insurance Rating Organization of Japan was used, and the selection criteria were as follows: (1) injuries resulting from a motor vehicle accident (rear-end collision, contact, or impact), (2) primary diagnosis of lumbar sprain, (3) medical treatment covered by compulsory automobile liability insurance, (5) compensation using compulsory automobile liability insurance completed between June 2014 and December 2019, and (6) damages due to injury of 400,000 yen or less with no permanent disability (a minor category in Japan's no-fault government compensation), or damages due to injury of less than 1,200,000 yen with permanent disability (a minor permanent disability in Japan's no-fault government compensation).

除外基準/Key exclusion criteria

本試験は損害保険料率算出機構から提供される自賠責保険を用いて頸椎捻挫、腰椎捻挫の治療を行った症例の請求費に関する国内全数調査です。
医療機関、医業類似施設のそれぞれで施設間の医療費の集中、偏在の実態を調査し、今後の保険診療のあり方、社会保障のあり方について検討をすることを目的としております。

This study is a nationwide analysis of claims data for cervical and lumbar sprains treated under the compulsory automobile liability insurance system in Japan, using data provided by the General Insurance Rating Organization of Japan.
The aim of this study is to examine the concentration and variation of medical costs across medical institutions and related care providers, and to inform future policy on insurance-covered healthcare and social security systems.
