本研究の目的は、北里大学医学部4年生および5年生を対象として、通常の精神科臨床実習に追加のアンチスティグマ教育介入を実施することが、医学生の精神疾患に対するスティグマに与える影響とその持続性を、実習開始時（T0）、実習終了時（T1）、および実習終了6か月後（T2）において検討することである。副次的には、精神疾患に対する知識、スティグマ態度、行動意図，共感性、援助希求性への影響とその持続性を検討する。さらに、背景要因との関連を探索的に検討するとともに、自由記述回答を用いて教育内容に対する主観的経験や態度変化の背景を質的に補足する。

The objective of this study is to examine whether an additional anti-stigma educational intervention during psychiatric clinical clerkship affects stigma toward mental illness among fourth- and fifth-year medical students at Kitasato University School of Medicine, and whether these effects are sustained from baseline (T0) to the end of the clerkship (T1) and 6 months after completion of the clerkship (T2). Secondary objectives are to evaluate the effects of the intervention on knowledge about mental illness, stigmatizing attitudes, behavioral intentions, empathy, and help-seeking, as well as the durability of these changes. In addition, the study will explore associations with background factors and qualitatively supplement the findings using free-text responses regarding subjective experiences of the educational content and the reasons for attitudinal changes.

介入群：通常の精神科臨床実習に加え、実習最終日に追加のアンチスティグマ教育介入を実施する。通常の精神科臨床実習には、担当入院患者の診察・面接、精神疾患に対するスティグマに関する基礎講義とヴィネットを用いた小討論（実習初日、30分）、および心理教育プログラムへの参加・見学（各90分×2回）が含まれる。追加のアンチスティグマ教育介入は、実習最終日（T1調査回答前）に約30分間実施し、精神疾患当事者がスティグマと回復体験を語る動画教材の視聴（約15分）および、実習中に経験したスティグマ関連場面についてのリフレクションワーク（個人記入とグループ共有、約15分）から構成される。

Intervention group: In addition to usual psychiatric clinical clerkship, participants receive an additional anti-stigma educational intervention on the final day of the clerkship. Usual psychiatric clinical clerkship includes daily interviews and examinations of assigned inpatients, a basic lecture on stigma toward mental illness and small-group discussion using a vignette on the first day of the clerkship (30 minutes), and participation in or observation of psychoeducation/group sessions (90 minutes each, twice). The additional anti-stigma educational intervention is delivered on the final day of the clerkship before the T1 assessment and lasts approximately 30 minutes. It consists of viewing a video in which a person with lived experience of mental illness talks about stigma and recovery (approximately 15 minutes) and a structured reflection exercise on stigma-related experiences during the clerkship, including individual worksheet completion and group sharing (approximately 15 minutes).

対照群：通常の精神科臨床実習のみを実施する。通常の精神科臨床実習には、担当入院患者の診察・面接、精神疾患に対するスティグマに関する基礎講義とヴィネットを用いた小討論（実習初日、30分）、および心理教育プログラムへの参加・見学（各90分×2回）が含まれる。追加のアンチスティグマ教育介入は行わない。なお、教育的配慮として、実習終了6か月後（T2）調査終了後に、介入群で用いた動画教材リンクおよびリフレクションワークシートを提供する。

Control group: Participants receive usual psychiatric clinical clerkship alone. Usual psychiatric clinical clerkship includes daily interviews and examinations of assigned inpatients, a basic lecture on stigma toward mental illness and small-group discussion using a vignette on the first day of the clerkship (30 minutes), and participation in or observation of psychoeducation/group sessions (90 minutes each, twice). No additional anti-stigma educational intervention is provided during the study period. For educational fairness, after completion of the 6-month follow-up assessment (T2), participants are provided with the same video material link and reflection worksheet used in the intervention group.

2026年3月23日から2028年3月3日までの期間に北里大学医学部の精神科臨床実習を受けた4年生または5年生のうち、研究の目的および方法等について説明を受け、文書による同意を提供した者。

Fourth- or fifth-year medical students at Kitasato University School of Medicine who participate in psychiatric clinical clerkship between March 23, 2026 and March 3, 2028, and who receive an explanation of the study objectives and procedures and provide written informed consent.

除外基準/Key exclusion criteria

1）過去に北里大学医学部の精神科臨床実習を受けたことがある者。
2）実習開始時（T0）の自己記入式質問紙に未回答の者。
3）実習期間中に1週間以上欠席または中断した者。
4）研究参加への同意を撤回した者。

1) Students who have previously completed psychiatric clinical clerkship at Kitasato University School of Medicine.
2) Students with no response to the self-administered questionnaire at baseline (T0).
3) Students who are absent from or discontinue the clerkship for 1 week or longer during the clerkship period.
4) Students who withdraw consent to participate in the study.
