本研究の目的は、進行消化器がん一次緩和化学療法導入期の患者を対象として、LIMとSICGを統合した二段階のコミュニケーション介入が、通常ケアと比較して12週（T3）時点のスピリチュアル・ウェルビーイング（SWB）を改善するかをランダム化比較試験により検証することである。

・第1段階：看護師によるLIM面談により、患者の人生経験を振り返り、価値・意味・信念を言語化する
・第2段階：患者・家族・看護師によるSICG面談にて、患者の価値観をふまえた療養について話し合う
・フォローアップ：介入後12週および24週時点で、短時間の電話フォローを実施し、価値に基づく療養の継続的な対話を支援する

Step 1 Life Review Interview Method (LIM)
A nurse conducts a LIM interview to help the patient reflect on their life experiences and articulate their values, meaning, and beliefs.

Step 2 Serious Illness Conversation Guide (SICG)
A conversation involving the patient, family members, and a nurse is conducted using the SICG to discuss future care and treatment in light of the patient's values.

Follow-up
Brief telephone follow-ups are conducted at 12 and 24 weeks after the intervention to support ongoing conversations about care consistent with the patient's values.

選択基準/Key inclusion criteria

日本語
1) 根治的治療が不能な転移性または再発消化器がんと診断された者。
2) 病理組織学的に胃がん、食道がん、胆道がん、膵臓がん、大腸がんのいずれかと診断されている者（組織型は問わない）。
3) 京都大学医学部附属病院で緩和目的の一次化学療法開始後６週間（42日）以内であり、外来通院が可能な者。
4) 病名および緩和的治療について主治医から説明を受けている者。
5) 同意取得時において年齢が20歳以上である者。
6) ECOG Performance Status（PS）が0から2である者。
7) 研究参加について、本人から文書による同意が得られている者。

1. Patients diagnosed with metastatic or recurrent gastrointestinal cancer that is not amenable to curative treatment.
2. Histologically confirmed gastric, esophageal, biliary tract, pancreatic, or colorectal cancer (any histological subtype).
3. Patients who have initiated first-line palliative chemotherapy at Kyoto University Hospital within 6 weeks (42 days) and are able to attend outpatient visits.
4. Patients who have been informed of their diagnosis and the palliative intent of treatment by their treating physician.
5. Age more than or equal to 20 years at the time of consent.
6. Eastern Cooperative Oncology Group (ECOG) Performance Status of 0 to 2.
7. Written informed consent obtained from the patient.

除外基準/Key exclusion criteria

日本語
1) つらさと支障の寒暖計（Distress and Impact Thermometer：DIT）で、つらさの点数が4点以上かつ支障の点数が3点以上の者。
2) 本研究への参加前に、主治医または看護師と病状認識の確認、予後に関する情報共有、価値観の確認、希望する医療やケアに関する話し合いのすべてを網羅した、体系的な対話を既に経験している者。
3) 主治医や看護師との医療やケアの方針に関する話し合いに、家族等の重要他者を一度も同伴することができない者。
4) 高次脳機能障害（明らかな知的機能低下、注意障害、記銘力障害）を有する者。
5) 日本語による会話および読み書きが不可能な者。
6) その他、主治医が本研究を実施するのに不適当と判断した者。

1. Patients with clinically significant distress, defined as a score of more than or equal to 4 on the distress scale and more than or equal to 3 on the impact scale of the Distress and Impact Thermometer (DIT).
2. Patients who have already experienced a structured conversation with their treating physician or nurse covering illness understanding, prognostic information sharing, clarification of personal values, and discussion of preferred medical care and treatment before enrollment in this study.
3. Patients who are unable to have a family member or other significant person accompany them to discussions regarding medical care or treatment decisions with their physician or nurse.
4. Patients with significant cognitive impairment, including evident cognitive impairment, attention deficits, or memory impairment.
5. Patients who are unable to communicate, read, or write in Japanese.
6. Patients whom the treating physician judges to be inappropriate for participation in this study for any other reason.
