本研究の目的は、日本の院内感染サーベイランス（JANIS）データベースを用いて整形外科手術後の手術部位感染（SSI）の全国的な季節変動を明らかにし、術後早期の気象条件、特に絶対湿度および気温とSSI発生との関連を検討することである。

The aim of this study is to investigate nationwide seasonal variation in surgical site infection (SSI) after orthopedic surgery in Japan and to evaluate the association between early postoperative meteorological conditions, particularly absolute humidity and temperature, and the risk of SSI using the Japan Nosocomial Infections Surveillance (JANIS) database.

主要アウトカム評価項目/Primary outcomes

整形外科手術後の手術部位感染（SSI）の発生。JANISの定義に基づき、表層SSIは術後30日以内、深部または臓器・体腔SSIはインプラントなしの場合は30日以内、インプラントありの場合は術後1年以内に発生したものと定義する。

Occurrence of surgical site infection (SSI) after orthopedic surgery as recorded in the Japan Nosocomial Infections Surveillance (JANIS) database. Superficial incisional SSI occurring within 30 days after surgery and deep incisional or organ/space SSI occurring within 30 days (without implant) or within 1 year (with implant) were included according to standard surveillance definitions.

整形外科手術症例のうち、2012年1月から2025年4月までに日本院内感染サーベイランス（JANIS）データベースに登録された症例を対象とする。

Orthopedic surgical procedures recorded in the Japan Nosocomial Infections Surveillance (JANIS) database between January 2012 and April 2025 will be included.

除外基準/Key exclusion criteria

日本語
主要多変量解析に必要な変数（年齢、性別、手術分類、手術時間、創分類、ASA分類、緊急手術の有無、インプラント使用の有無、施設識別子、手術日）の情報が欠損している症例は除外する。

Procedures with missing data for variables required in the primary multivariable analysis, including age, sex, surgery type, operative time, wound class, ASA classification, emergency surgery status, implant use, hospital identifier, or surgery date, will be excluded.

本研究は、日本院内感染サーベイランス（JANIS）データベースを用いた全国規模の後ろ向き観察研究である。2012年1月から2025年4月までに登録された整形外科手術症例を対象とし、手術部位感染（SSI）の発生率の季節変動を評価するとともに、術後早期の気象条件（特に絶対湿度および気温）との関連を解析する。解析では患者背景因子および手術関連因子を調整した多変量モデルを用いる。

This study is a nationwide retrospective observational study using the Japan Nosocomial Infections Surveillance (JANIS) database. Orthopedic surgical procedures recorded between January 2012 and April 2025 will be analyzed to evaluate seasonal variation in surgical site infection (SSI) and its association with early postoperative meteorological conditions, particularly absolute humidity and temperature. Multivariable models adjusted for patient and surgical characteristics will be used.
