低カリウムレタス 100 g を、試験期間中(Trial 1:4 日間、Trial 2:透析間隔中)に 1 日 1 回摂取する。レタスは生食とし、その他の食事制限は通常の指導に従う。

低カリウムレタス群では、介入後に 血清カリウム値が有意に低下した。
通常レタス群では有意な変化は認められなかった。
群間比較では、低カリウムレタス群の方が血清カリウム低下量が有意に大きかった。
副次評価項目(安全性)
介入期間中、有害事象は認められず、消化器症状や腎機能悪化も発生しなかった。
血圧、体重、尿検査値にも臨床的に問題となる変化はなかった。
結論
低カリウムレタスは、非透析慢性腎臓病患者において安全に摂取でき、血清カリウム値を低下させる効果があることが示された。

Although the final analysis of this trial has been completed, preparation of the manuscript for journal submission has required additional time, resulting in delayed reporting on UMIN. Interim results have already been presented at a scientific conference, and the final results will be disclosed after submission to the international journal Nutrients.

参加者は非透析の慢性腎臓病(CKD)患者であり、介入群(低カリウムレタス群)と対照群(通常レタス群)の背景因子は均衡していた。平均年齢はそれぞれ 67±10 歳、66±11 歳で、男性は 58% と 60% であった。BMI は 23.4±3.1 と 23.1±3.4 で差はなかった。腎機能(eGFR)は 38±12 と 39±11 mL/minで同等であり、血清カリウム値は両群とも 4.8 mEq/L 前後であった。栄養指標(アルブミン 4.1 g/dL)や炎症マーカー(CRP 約 0.1 mg/dL)にも群間差は認められなかった。原疾患は糖尿病性腎症と腎硬化症が中心であり、併存症として高血圧・糖尿病が多く、両群で同程度であった。

Participants were non dialysis chronic kidney disease (CKD) patients, and baseline characteristics were well balanced between the intervention (low potassium lettuce) and control groups.

適格性評価のために 10 名をスクリーニングし、全員が登録された。10 名はランダム化され、通常レタス群 5 名、低カリウムレタス群 5 名に割り付けられた。通常レタス群では 1 名が利尿薬開始のため中止し、4 名が介入を完了した。低カリウムレタス群は 5 名全員が介入を完了した。最終的に 10 名が ITT 解析に含まれ、介入を完了した 9 名が PP 解析に含まれた。

Individual participant data (IPD) collected in this study will not be shared with third parties due to concerns regarding personal privacy. Clinical variables such as serum potassium levels, renal function parameters (eGFR and serum creatinine), and urinary sodium excretion may allow potential identification of participants even after anonymization; therefore, IPD will not be made publicly available. However, anonymized aggregate data may be provided upon reasonable request for the purpose of secondary analysis or verification of the study findings. The principal investigator will review each request, and data will be shared only when it is deemed ethically appropriate and when participant confidentiality can be fully protected.
