目的1/Narrative objectives1

日本語
片麻痺患者に対する長下肢装具（KAFO）を使用した前型交互歩行（AGT）において、患者自身のペース（PP-AGT）と介助者（理学療法士）優位のペース（TL-AGT）のどちらが歩行能力を改善させるかを明らかにする。本予備調査では、本研究のための正確なサンプルサイズ計算に必要な情報を取得することを目的とする。

The purpose of this pilot study is to clarify whether patient-paced alternate gait training (PP-AGT) or therapist-led alternate gait training (TL-AGT) is more effective in improving gait ability in stroke patients using knee-ankle-foot orthoses (KAFO). The results will be used to obtain information necessary for calculating the accurate sample size for a subsequent main study.

介入1：患者ペースの交互歩行練習（PP-AGT）
内容： ゲイトソリューション足継手付きの長下肢装具（KAFO）を使用し、膝継手は伸展位でロックして実施する。両下肢を交互に振り出す前型歩行を行う。
個別条件： 平行棒および杖の使用を可能とする。歩行のペースは対象者主体（患者自身のペース）とし、理学療法士は歩行動作が成立するような最小限の介助を行う。
プロトコル： 週5回以上、1回あたり合計10分以上実施する。

Intervention 1: Patient-Paced Alternate Gait Training (PP-AGT)
Description: Walking training is performed using a knee-ankle-foot orthosis (KAFO) with a Gait Solution ankle joint and the knee joint locked in extension. The training involves an alternate gait pattern, stepping forward with each leg alternately.
Specific Conditions: The use of parallel bars or a cane is allowed. The walking pace is determined by the participant (patient-paced), and the physical therapist (PT) provides only the minimal assistance necessary to maintain the gait movement.
Protocol: At least 5 sessions per week, with a total duration of 10 minutes or more per session.

Intervention 2: Therapist-Led Alternate Gait Training (TL-AGT)

選択基準/Key inclusion criteria

脳卒中片麻痺患者
入院時のFIM歩行項目が1点（全介助）であり、歩行練習に介助を要する者
長下肢装具（KAFO）の使用条件を満たす以下の身体状況にある者：
短下肢装具（AFO）を使用しても、膝折れや反張膝（バックニー）が生じるなど、適切に歩行練習が実施できない者
下肢に前型交互歩行（AGT）の動作を阻害するような関節可動域制限（拘縮など）を認めない者
中等度から重度の運動麻痺（Stroke Impairment Assessment Set [SIAS] 膝運動スコア3以下）を呈する者
担当理学療法士が本研究への参加を妥当と判断し、了承したもの

Stroke patients with hemiplegia
Patients requiring total assistance for walking at admission (Functional Independence Measure [FIM] gait score of 1)
Patients meeting the following criteria for requiring a knee-ankle-foot orthosis (KAFO):
Inability to walk appropriately with an ankle-foot orthosis (AFO) due to physical issues such as knee buckling or hyperextension (genu recurvatum)
Absence of joint range of motion (ROM) limitations in the lower limbs that would hinder alternate gait training (AGT)
Presence of moderate to severe motor paralysis, defined as a Stroke Impairment Assessment Set (SIAS) knee motor score of 3 or less
Patients whose participation is approved by the physical therapist in charge
