免疫チェックポイント阻害薬の治療効果予測因子として、腫瘍微小環境における三次リンパ様構造（TLS）が注目されているが、従来の鉗子生検では検体の挫滅により正確な評価が困難であった。本研究では、大きな検体採取が可能で組織構造の保存に優れる経気管支クライオ生検（TBCB）を用い、同一患者の手術検体との対比を行うことで、TBCBによるTLS評価の診断妥当性を検証する [cite: 31, 71]。これにより、術前の段階での新たなバイオマーカー評価法としての確立を目指す。

Tertiary lymphoid structures (TLS) in the tumor microenvironment are attracting attention as predictive factors for the efficacy of immune checkpoint inhibitors, but accurate evaluation has been difficult with conventional forceps biopsy due to specimen crushing[cite: 31]. This study uses transbronchial cryobiopsy (TBCB), which allows for larger specimen collection and superior preservation of tissue architecture, to validate the diagnostic validity of TLS evaluation by comparing it with surgical specimens from the same patient. The goal is to establish TBCB as a novel pre-operative biomarker assessment method.

当院において、術前診断目的に経気管支クライオ生検（TBCB）が施行された非小細胞肺癌患者

TBCB施行後、原発巣の外科的切除が行われた患者

解析に必要な診療録情報および保存病理検体（パラフィンブロック）の使用が可能な患者

Patients with non-small cell lung cancer who underwent transbronchial cryobiopsy (TBCB) for preoperative diagnosis at our hospital

Patients who subsequently underwent surgical resection of the primary lesion

Patients whose medical records and preserved pathological specimens (paraffin blocks) are available for analysis

除外基準/Key exclusion criteria

日本語
採取された検体量が不十分であり、病理学的な評価（TLSの有無および成熟度）が困難な症例

術前に化学療法または放射線療法が施行された症例

その他、研究責任者が本研究の対象として不適当と判断した症例
