本研究の目的は、社会人ラグビー選手におけるスポーツ外傷・障害に対する鍼治療の直後効果を検討することである。また、副次的評価として、当該チームにおいて発生したスポーツ外傷・障害の傾向を疫学的に把握し、鍼治療の直後効果が社会人ラグビーチームに貢献し得るかを探索的に検討する。社会人ラグビーチームの疫学調査により、当該チームのスポーツ外傷・障害の傾向が明らかとなり問題点を把握することができる。スポーツ外傷・障害の状況や発生率、離脱日数の負担度が高い疾患に対して鍼治療が貢献するかを単群前後比較（介入研究）による痛みの評価や効果と照合することにより社会人ラグビーチームに貢献できるかを明らかにすることができる。

The purpose of this study is to examine the immediate effects of acupuncture on sports-related injuries and disorders in adult rugby players. As a secondary objective, this study aims to epidemiologically investigate the patterns of sports-related injuries and disorders occurring within the team and to explore whether the immediate effects of acupuncture may contribute to the management of injuries in an adult rugby team.

Through an epidemiological survey of the adult rugby team, the characteristics and trends of sports-related injuries and disorders will be clarified, enabling identification of key clinical issues. By comparing the incidence and time loss duration of specific injuries with the immediate effects of acupuncture assessed using a single arm pre-post intervention design, this study aims to determine whether acupuncture may contribute to the management of sports-related injuries in adult rugby players.

主要アウトカム評価項目/Primary outcomes

痛みのVisual Analogue Scale（以下、VAS）。VASは、長さ10cmの黒い線（左端が「痛みなし」、右端が「想像できる最大の痛み」）のうち、現在の痛みがどの程度かを指し示す視覚的なスケールである。約2年間の間に発生したラグビー選手のスポーツ外傷・障害のうち離脱を伴うTime Loss Injuryに対して鍼治療を行い、その前後でVASの評価を行う。部位別、スポーツ外傷・障害別にVASの効果の違いを比較検討する。

Pain intensity assessed using the Visual Analogue Scale (VAS). The VAS consists of a 10-cm horizontal line anchored by "no pain" on the left end and "the worst imaginable pain" on the right end, on which participants indicate their current level of pain.

Acupuncture will be administered to rugby players with time-loss injuries (defined as sports-related injuries or disorders resulting in absence from participation) that occurred over an approximately 2-year period. Pain intensity will be evaluated using the VAS immediately before and after acupuncture treatment.

Changes in VAS scores will be compared according to injury location and type of sports-related injury or disorder.

疫学指標（発生率、重症度、負担度など）。約2年間の間に社会人ラグビーチームで生じたスポーツ外傷・障害に対して疫学指標を用いて調査する。その後、鍼治療を行ったスポーツ外傷・障害の特性を明らかにする。
調査する疫学指標は以下の通りである。
1．スポーツ外傷・障害受傷者数　
2．スポーツ外傷・障害発生数　
3．Athlete-exposure
4．Athlete-hour　5．Prevalence（有病率：%）　
6．Incidence（発生率：件/1,000athlete-exposure、件/1,000athlete-hours）
7．Injury risk ratio（外傷・障害リスク比）　
8．Injury rate ratio（外傷・障害率比：1,000/athlete-exposures、1,000/athlete-hours）
9．Injury burden（外傷・障害の負担：incidence (athlete-exposure、athlete-hour）×重症度)

Epidemiological indicators (e.g., incidence, severity, and burden) will be used to investigate sports-related injuries and disorders occurring in an adult rugby team. In addition, the characteristics of sports-related injuries and disorders treated with acupuncture will be clarified.

The following epidemiological indicators will be assessed:

1.Number of injured players with sports-related injuries and disorders
2.Number of injury events
3.Athlete exposure
4.Athlete hours
5.Prevalence (percent)
6.Incidence (cases per 1000 athlete exposures and cases per 1000 athlete hours)
7.Injury risk ratio
8.Injury rate ratio (per 1000 athlete exposures and per 1000 athlete hours)

選択基準/Key inclusion criteria

社会人ラグビーチームA所属の約５０名のラグビー選手および離脱を伴うスポーツ外傷・障害を受傷した選手とする。
鍼治療の介入する組み入れ基準は
「ドクター及びトレーナーが適応を判断し、選手の同意が得られたもの」とする。

組み入れ基準
24時間を超えて練習に参加できない筋骨格系の運動器症状の痛みを有するもの

Approximately 50 rugby players belonging to adult rugby team A will be included in this study. Participants will be rugby players who sustained sports-related injuries or disorders resulting in time loss from participation.

The inclusion criteria for acupuncture intervention are cases in which the team doctor and trainer determine that acupuncture is appropriate and the player provides informed consent.

Inclusion criteria

Players who have musculoskeletal pain associated with sports-related injuries or disorders that prevent participation in training for more than 24 hours.

Suspected acute traumatic injuries such as head or neck injuries, fractures, dislocations, or severe ligament injuries.

Presence of severe neurological findings.

Presence of severe inflammatory findings.

Suspected internal medical diseases or infectious diseases.

Cases judged to be inappropriate for acupuncture treatment by the physician.
