日本語
本研究の目的は、前向き研究により外来心臓リハビリテーション（主に運動療法）への参加に関する意識調査を実施し、参加の障壁を明らかにして個別化支援に活用することである。さらに、実際の参加・継続状況と各障壁との関連を検討し、得られた知見を実臨床に還元することを目指す。

The aim of this study is to conduct a prospective survey of patients attitudes toward participation in outpatient cardiac rehabilitation, to identify barriers to participation, and to utilize this information for individualized support. Furthermore, we aim to examine the relationships between actual participation/continuation status and each barrier and to feed the findings back into real-world clinical practice.

以下のすべての条件を満たす患者
① 年齢が20歳以上
② 急性あるいは慢性心不全急性増悪にて春日井市民病院循環器内科に入院治療を要した
③ 入院時に介護保険未申請

Patients who meet all of the following criteria:
Age>20 years
Require inpatient treatment in the Department of Cardiology at Kasugai Municipal Hospital for acute heart failure or acute exacerbation of chronic heart failure
Not enrolled in Long-term Care Insurance at the time of admission
除外基準/Key exclusion criteria

以下のいずれかに該当する患者
① 高度難聴や認知症にてコミュニケーション困難
② 調査票による介入を拒否
③ 研究責任医師、あるいは研究分担医師が不適当と判断

Severe hearing impairment or dementia that makes communication difficult
Refusal to participate in the questionnaire-based intervention
Unsuitable by the principal investigator or a co-investigator
