日本語
肺移植の周術期および術後に得られる検体を用いて、重症慢性進行性肺疾患（ならびに移植後の慢性呼吸機能障害を含む）の病態を臨床情報と統合して解析し、病態の層別化や臨床経過との関連を明らかにすることを目的とする。

The aim of this study is to analyze the pathophysiology of severe progressive chronic lung diseases, including post-transplant chronic respiratory dysfunction, by integrating perioperative and postoperative samples obtained from lung transplantation with clinical data, and to clarify disease stratification and its association with clinical outcomes.

慢性移植肺機能不全（CLAD）の発症（有無）および発症までの期間、全生存および移植後生存期間、primary graft dysfunction（PGD）の発症（有無）および重症度、急性拒絶反応の発症（有無）、抗HLA抗体（DSA）の出現（有無）、感染症イベント（有無）、ならびに検体由来指標〔酸化ストレス指標、遺伝子変異、遺伝子発現（RNA）・タンパク発現〕と臨床経過・予後との関連（探索的）

Development of chronic lung allograft dysfunction (CLAD) (incidence and time to onset), overall survival and post-transplant survival, incidence and severity of primary graft dysfunction (PGD), incidence of acute rejection, development of donor-specific anti-HLA antibodies (DSA), infectious events, and exploratory associations between clinical outcomes/prognosis and sample-derived markers, including oxidative stress markers, genetic variants, and gene (RNA) and protein expression profiles.

対象疾患：肺高血圧症、特発性間質性肺炎、肺気腫、造血幹細胞移植後肺障害等、日本肺および心肺移植研究会における肺移植レシピエント適応基準に準じた疾患
・すべての年齢と男女を対象とする
・名古屋大学医学部附属病院呼吸器外科・呼吸器内科受診者のうち、同院で肺移植適応評価入院を受けた方
・当院で肺移植登録が行われた方
・名古屋大学医学部附属病院呼吸器外科において肺移植手術を受けた方
・診療情報の閲覧と研究利用に同意いただける方
・本人が本研究の内容を理解し、文書による同意が得られた方
・「慢性間質性肺疾患のアウトカムに関連する新規疾患指標の探索研究」(承認番号2017-0169)にて検体やカルテデータ等の利用に関する包括同意を得た方

Target diseases include pulmonary hypertension, idiopathic interstitial pneumonia, pulmonary emphysema, lung disorders after hematopoietic stem cell transplantation, and other diseases that meet the lung transplant recipient eligibility criteria defined by the Japanese Society of Lung and Heart-Lung Transplantation.
Patients of any age and sex.
Patients who visited the Departments of Thoracic Surgery or Respiratory Medicine at Nagoya University Hospital and underwent inpatient evaluation for lung transplantation at the same institution.
Patients who were registered for lung transplantation at Nagoya University Hospital.
Patients who underwent lung transplantation surgery at the Department of Thoracic Surgery, Nagoya University Hospital.
Patients who agreed to allow access to and research use of their clinical information.
Patients who were able to understand the content of this study and provided written informed consent.
Patients who had previously provided broad consent for the use of biological samples and clinical data under the study entitled "Exploratory Study of Novel Disease Markers Associated with Outcomes of Chronic Interstitial Lung Diseases" (Approval No. 2017-0169).

Patients who are unable to adequately understand the content of the study and are therefore unable to provide written informed consent.
Patients for whom it is difficult to obtain written informed consent directly from the individual.
Patients who are judged by the attending physician to be inappropriate for enrollment in this study for any reason.
