目的1/Narrative objectives1

日本語
大腿骨近位部骨折は高齢者に多く，術後早期からの荷重・歩行開始が求められる．術後炎症や急性疼痛は患側荷重を阻害し歩行獲得を遅延させる．したがって，炎症や疼痛を軽減し荷重を促進する手段が求められる．アイシングは疼痛・炎症の軽減に有効とされるが，運動療法直前の適用が荷重率や歩行能力に及ぼす影響を検証した報告はない．本研究は，運動療法直前のアイシングが患側荷重率，荷重時痛，運動機能，歩行能力，CRP値に与える影響を明らかにすることを目的とした．

Proximal femoral fractures are common in the elderly, requiring early weight-bearing and walking initiation postoperatively. Postoperative inflammation and acute pain inhibit weight-bearing on the affected side and delay walking acquisition. Therefore, methods to reduce inflammation and pain while promoting weight-bearing are needed. Icing is considered effective for reducing pain and inflammation, but no reports have examined its effects on weight-bearing rate and walking ability when applied immediately before exercise therapy. This study aimed to clarify the effects of icing immediately before exercise therapy on weight-bearing ratio, weight-bearing pain, motor function, walking ability, and CRP levels.

アイシングは純氷500 g に対し50 mL の水を加えた湿潤氷を使用し，適用部位は創部直上，理学療法開始直前に4日間連続で実施した．介入頻度は1日1回で，1回のアイシング時間は20分間とした．

Icing was performed using wet ice made by adding 50 mL of water to 500 g of pure ice. The application site was directly above the wound, and icing was administered for four consecutive days immediately before the start of physical therapy. The intervention frequency was once daily, with each icing session lasting 20 minutes.

コントロール群はリハビリ直前のアイシングを実施しなかったが，病棟での一般的なアイシング（疼痛管理目的）は両群とも同様に許可された．

2025年4月から，同年11月までに大腿骨近位部骨折に対して人工骨頭置換術もしくは髄内釘固定を施行された術後急性期患者とした．

Patients undergoing acute postoperative care who underwent total hip arthroplasty or intramedullary nailing for proximal femoral fractures between April and November 2025.

除外基準/Key exclusion criteria

日本語
除外基準は，術側の膝・足関節に疾患既往があるもの，血液データに影響を及ぼすと思われる疾患があるもの，術後に荷重制限が生じたものとした．

Exclusion criteria were defined as having a history of disease in the knee or ankle joint on the operative side, having a disease likely to affect blood data, or having weight-bearing restrictions postoperatively.
