目的1/Narrative objectives1

日本語
帯状疱疹発症患者を対象に、帯状疱疹後神経痛発症に至るリスク因子を前向きに検討し、特に患者のフレイル状態が帯状疱疹後神経痛発症に及ぼす影響を明らかにすることである。これにより、帯状疱疹後神経痛のリスク層別化や予防的介入の科学的根拠を確立し、高齢者における疼痛管理と生活機能維持に寄与することを目指す。

The aim of this study is to prospectively investigate risk factors leading to the development of postherpetic neuralgia (PHN) among patients with herpes zoster, with a particular focus on elucidating the impact of patient frailty status on the onset of PHN. By doing so, we seek to establish scientific evidence for PHN risk stratification and preventive interventions, ultimately contributing to improved pain management and the maintenance of functional capacity in older adults.

帯状疱疹後神経痛は帯状疱疹の主要合併症であり、長期的な疼痛治療やリハビリを要するため生活の質を著しく低下させる。高齢、女性、免疫低下、皮疹重症度は基地の危険因子だが、患者フレイルが帯状疱疹後神経痛への移行に与える影響については十分に検証されていない。フレイルは老化や慢性炎症、疼痛感受性亢進により神経障害性疼痛を遷延させる可能性がある。本研究は、前向き観察によりフレイルの指標を含めた急性期因子を評価し、多変量解析によって独立危険因子を明らかにする。
本研究で行う問診や非侵襲的検査が帯状疱疹後神経痛発症に与える影響を評価することで、帯状疱疹後神経痛のリスク層別化が可能になれば帯状疱疹ワクチン接種などの予防的介入や帯状疱疹発症後の早期疼痛管理など個別化治療戦略の構築に寄与する。

Postherpetic neuralgia (PHN) is a major complication of herpes zoster and markedly reduces quality of life due to prolonged pain management needs. Although age, female sex, immunosuppression, and rash severity are known risk factors, the role of frailty in PHN development remains unclear. Frailty may prolong neuropathic pain through aging-related mechanisms, chronic inflammation, and increased pain sensitivity.
This prospective study will assess acute-phase factors including frailty and identify independent predictors using multivariable analysis. Establishing PHN risk stratification may support preventive strategies such as vaccination and early individualized pain management.

・帯状疱疹発症後3か月以内で同意取得時に18歳以上の患者
・研究の参加に同意が得られる患者

Patients aged 18 years or older who are within 3 months of herpes zoster onset at the time of consent

Patients who are able to provide informed consent to participate in the study

除外基準/Key exclusion criteria

日本語
・すでに慢性疼痛と診断されている患者のうち、初診時疼痛スクリーニング検査で疼痛背景が帯状疱疹に関連するか判断が困難な患者
・追跡が困難な患者
・既往により下肢・測定部位に変形や手術歴があり正確に歩行検査や筋厚測定が受けられない患者

Patients already diagnosed with chronic pain in whom the initial pain screening makes it difficult to determine whether the pain is related to herpes zoster

Patients who are expected to be difficult to follow up

Patients with deformities or a history of surgery in the lower limbs or measurement sites that prevent accurate gait assessments or muscle thickness measurements
