一般的にNICU・GCU病棟では早産・超低出生体重児が多く入院しており、入院児の母が搾乳した母乳を取り扱っている。母乳は壊死性腸炎、未熟児網膜症、慢性肺疾患などの罹患率を減らし、感染症の発症率を減らす(長屋建：Neonatal　Care2018　Vol.31　no.7(631))。ひいては生命予後や神経発達予後に大きく影響するものであり、必要不可欠で非常に大切に扱うべきものである。東邦大学医療センター大森病院2号館8階東病棟はNICU(新生児集中治療室)・GCU(新生児治療回復室)病棟である。母乳を常温・冷蔵・冷凍で3種類に分けて保管しており、保存期限がそれぞれ異なる。必要量と使用期限を確認して準備をする必要があり、間違えると廃棄に繋がる。母乳の投与方法に関しても、直接授乳・定時授乳・不規則定時授乳・自律授乳など多岐にわたることが特徴である。そのため、看護師が母乳を取り扱う場面が多く煩雑であり母乳廃棄に至るリスクが高い。また、母乳を扱う産科病棟や小児病棟と比較しても、保管方法・期限・投与方法の種類が多く他病棟との比較が出来ず母乳の取り扱いに関する事故の予防策の共有が難しい。
「母乳の取り扱い」に関する病棟独自の基準手順として「母乳を分ける作業」、「ミルク・母乳の投与」と2つがある（以下「母乳の取り扱い」とする）。インシデントが起こる度、病棟内で適宜基準手順の見直しを行い、院内メールや朝礼時に周知しているものの、周知内容の把握は各スタッフの判断に任されており、教育体制が不十分であった。
そのため再発防止策は具体的な対策案が必要となると考えた。2号館8階東病棟では、基準手順の改定を行っているものの、インシデント・アクシデントの報告は年々増加傾向にある。過去1年間の中で、「療養上の世話」における66件のインシデント・アクシデント中、「母乳の取り扱い」に関する事例が54件(81．0％)であった。その中でも母乳を廃棄することとなった事例は39件であり、72％を占めている。基準手順が遵守されていない事例は34件あり、病棟看護師において基準手順が守られていないことが多いということが分かった。
「母乳の取り扱い」に関する事故に対して、2号館8階東病棟看護師の基準手順の遵守率を向上させるための教育的介入を行うことで行動の変化を明らかにし、教育的介入効果の示唆を得ることを目的とし、患者の母乳栄養を守ることにつなげたい。

In neonatal intensive care units (NICU) and growing care units (GCU), many infants are premature or extremely low birth weight. Nurses frequently handle expressed breast milk provided by mothers. Breast milk is essential in neonatal care because it reduces necrotizing enterocolitis, retinopathy of prematurity, chronic lung disease, and infections (Nagai Tate, Neonatal Care, 2018, Vol.31, No.7, p.631). Therefore, it must be handled safely. The 8th East Ward of Building No.2 at Toho University Medical Center Omori Hospital serves as both a NICU and a GCU. Breast milk is stored at room temperature, refrigerated, or frozen, each with different storage periods. Nurses must confirm the amount and expiration before preparation; mistakes may cause unnecessary disposal. Administration methods vary by infant condition, making procedures complex. Because of this, nurses in the NICU/GCU face a high risk of errors and waste. Compared with maternity or pediatric wards, storage and administration methods are more diverse, making it difficult to share preventive measures. The ward has two procedures for breast milk handling: (1) dividing and labeling expressed milk and (2) administering milk or breast milk. These are reviewed after incidents, but information sharing depends on each nurse, and education is insufficient. Over the past year, 66 nursing incidents were reported; 54 (81%) involved breast milk handling, and 39 (72%) led to disposal. In 34 cases, procedures were not followed, showing frequent noncompliance. This study aims to improve nurses adherence to procedures through educational intervention. By promoting understanding and behavioral change, we seek to reduce incidents and ensure safe, efficient management of breast milk for premature and low birth weight infants, improving patient safety and care quality.

教育的介入
1.動画視聴

「母乳の取り扱い」の基準手順を示す動画を作成（患者の個人情報は使用しない）。

Google クラスルームを利用して視聴してもらう。

2.小グループによる対面教育

研究メンバー以外の看護師を5～10人程度の小グループに分け、勤務終了後に実施。

内容：

遵守率が低かった手順の説明

チェックリストを用いた一問一答

遵守率が低い場面のデモンストレーション

3.参加の自由性

研究に同意していない者も教育的介入に参加可能。

動画視聴、一問一答、デモンストレーションへの参加は希望者の自由意志とする。

In the NICU (Neonatal Intensive Care Unit) and GCU (Growing Care Unit), many hospitalized infants are preterm or have extremely low birth weight. As a result, their mothers expressed breast milk must be handled within the units, which increases the likelihood of incidents such as disposal of breast milk or incorrect administration.

In the 8th East Ward of Building No2, ward nurses are responsible for handling the expressed breast milk of hospitalized infants. However, incidents such as disposal or incorrect administration of breast milk by these nurses have been reported.

This study targets the nurses of the 8th East Ward of Building No2, aiming to clarify behavioral changes resulting from an educational intervention and to evaluate the effectiveness of such an intervention.

47名中43名（91.4%）が同意し、全介入を受けた40名（93.0%）を解析対象とした。遵守率は全体で向上したが、デモ・一問一答未実施項目では低下した。事故は介入前8件（廃棄3件）から介入後13件（廃棄10件）へ増加。アンケートでは全員が遵守の必要性を認識し、介入は概ね妥当と評価されたが、手順の複雑さが事故要因として指摘された。

Of 47 eligible participants, 43 (91.4%) consented; data from 40 (93.0%) who completed all interventions were analyzed. Adherence to standard procedures increased overall, but decreased in items without demonstration or Q&A. Incidents rose from 8 (3 disposals) pre-intervention to 13 (10 disposals) post-intervention. Of 39 respondents, all recognized the need for adherence and rated the intervention appropriate, though procedural complexity was noted as a possible cause of incidents.
