本研究は、通所リハビリテーションを利用する地域在住高齢者を対象に、機能・活動性・生活の質（QOL）の縦断的変化を追跡し、その関連構造を明らかにするものである。特に、機能改善が生活空間（Life-Space Assessment）を介してQOLやIADLの改善に寄与する経路（メディエーション効果）を検証し、併せて、主観的改善感をアンカーとした最小臨床的重要差（MCID）の推定および機能・活動性プロファイルの分類を行う。これにより、通所リハの効果を可視化し、臨床・経営両面でのアウトカム評価と質改善につなげることを目的とする。

This study aims to longitudinally examine changes in functional status, activity, and quality of life among community-dwelling older adults attending day rehabilitation services. Specifically, it investigates the mediation pathway through which functional improvements contribute to better life-space mobility and consequently to enhanced health-related quality of life (HRQoL) and instrumental activities of daily living (IADL). In addition, the study estimates the minimal clinically important difference (MCID) using patient-perceived improvement as an anchor and identifies functional activity profiles associated with clinical outcomes. The ultimate goal is to provide evidence for the measurable impact of day rehabilitation, facilitating data-driven quality improvement in clinical practice and service management.

1.当院の通所リハビリテーションを新規または継続して利用する者
2.65歳以上の地域在住高齢者
3.研究内容について文書により同意を得られた者

1. Individuals who are newly enrolled in or currently attending the day rehabilitation program at our institution.
2. Community-dwelling older adults aged 65 years or older.
3. Participants who provide written informed consent after being informed of the study details.

除外基準/Key exclusion criteria

日本語
1. 認知機能障害や重度の身体疾患などにより、評価や質問紙調査への参加が著しく困難な者。
2. 主治医が本研究への参加を不適当と判断した者。
3. 追跡不能（死亡・長期入院・転居など）となった場合は、解析上は検閲（censoring）として扱い、事前除外は行わない。

1. Individuals with cognitive or severe physical conditions that make participation in assessments or questionnaire surveys substantially difficult.
2. Individuals deemed unsuitable for participation by their attending physician.
3. Loss to follow-up events such as death, long-term hospitalization, or relocation will be handled as censoring in the analysis rather than as exclusion at baseline.
