日本語
末梢留置型中心静脈カテーテル（PICC）の挿入先端位置は上肢の位置と呼吸に依存することが先行研究で判明しているが、日本人成人を対象とした研究は行われていない。また、PICCの先端の最適位置については「PICC」を「中心静脈カテーテル（CVC）」のひとつだとみなされ論じられていることが多く、厳密には使用目的や構造も異なるPICCをCVCと同様の先端最適位置にして良いのかを含め、現在も論文やガイドラインで意見が分かれており議論を呈している。
今回、日本人成人を対象とするPICC挿入先端位置の上肢位置と呼吸の変動に関して調べることで、日本人成人に特異的なPICC先端位置の変動を見つけ出し、日本人成人における最適なPICC挿入先端位置を決める際などの基準となる肢位や呼吸相の目安になり得る可能性が考えられる。

Previous studies have found that the insertion tip position of peripherally indwelling central venous catheters (PICCs) depends on the position of the upper limb and respiration, but no studies have been conducted in Japanese adults. The optimal position of the tip of the PICC is often discussed because the PICC is considered to be a central venous catheter (CVC), and there are still differences of opinion in articles and guidelines, including whether PICCs, which strictly speaking have different purposes and structures, should have the same optimal tip position as CVCs. The issue is still controversial, with differing opinions in papers and guidelines.
In this study, we investigated the upper limb position and respiratory variability of the PICC insertion tip position in Japanese adults to identify PICC tip position variability specific to Japanese adults, which may serve as a reference limb position and respiratory phase when determining the optimal PICC insertion tip position in Japanese adults. The results of this study are as follows.

所属施設の倫理審査委員会承認日から2026年3月31日までの間に、NP（Nurse Practitioner）室所属の医療安全委員会にて医師の指示のもと単独でPICCの挿入が認められた診療看護師（Nurse Practitioner: NP）が透視室にて施行する、各診療科医師より依頼された日本人成人で本研究に同意した、右上肢からのPICC（Groshong Catheter）挿入患者を対象とし、上肢位置・呼吸期によるPICC挿入先端位置の変動データを収集する。

The study was performed in the fluoroscopy room by a nurse practitioner (NP) who was authorized by the medical safety committee of the Nurse Practitioner (NP) office to insert a PICC alone under the direction of a physician, between the date of approval by the ethics review committee of his/her institution and 31 March 2026, for each of the following patients Data on the variation of the PICC insertion tip position according to upper limb position and respiratory phase will be collected in Japanese adult patients with PICC (Groshong Catheter) insertion from the right upper limb, who agreed to the study and were requested by a physician in the department of medicine.
