(1) 医学部長実施許可日から2027年3月31日までの期間に大分大学医学部附属病院および本研究に参加する医療機関を受診し、上部消化管内視鏡検査を受けるH. pylori未感染の胃腫瘍症例と胃腫瘍に対して内視鏡治療後の症例
(2) 口頭および文書による本研究参加への同意が得られた症例
(3) 年齢が18歳以上の症例

(1) H. pylori-naive gastric cancer patients who visit Oita University Hospital or one of the participating facilities and undergo an upper gastrointestinal endoscopy
(2) Patients who have given their consent to participate in this study, either verbally or in writing
(3) Patients who are over 18 years of age

除外基準/Key exclusion criteria

日本語
(1) H. pylori陽性の症例
(2) 過去にH. pylori除菌治療を受けた症例
(3) 内視鏡検査施行日の2週間以内に酸分泌抑制薬や抗菌薬を内服している症例
(4) 免疫抑制薬やステロイド薬を内服している症例
(5) 重篤な合併症（心不全、腎不全、肝不全、呼吸不全）を有する症例
(6) その他、本研究への参加に不適切と判断された症例

(1) Cases positive for H. pylori
(2) Cases with a history of H. pylori eradication therapy
(3) Cases taking acid inhibory drug or antibiotics within 2 weeks prior to the endoscopy date
(4) Cases taking immunosuppressants or steroids
(5) Cases with severe complications (heart failure, renal failure, liver failure, respiratory failure)
(6) Other cases deemed inappropriate for participation in this study

4)対象者：大分大学医学部長実施許可日から2027年3月31日までに大分大学医学部附属病院および本研究の共同研究施設（株式会社ミズホメディーを除く）の外来を受診したH. pylori未感染の胃腫瘍患者と胃腫瘍に対して治療後の患者で、今回の臨床研究への参加に同意が得られ、前述の適格基準をすべて満たし、除外基準のいずれにも該当しない参加者
5)検体の採取施設(14施設)：大分大学、順天堂大学、島根大学、京都医療センター、鳥取大学、国際医療福祉大学市川病院、富山大学、横浜市立大学、京都府立医科大学、湘南鎌倉総合病院、国立国府台医療センター、虎の門病院、北海道対がん協会、ふるた内科クリニック
6)対象症例数：150例
7)方法
(1)参加同意取得
(2)臨床情報の聴取: 一般的な問診と腹部症状のアンケートの実施
(3)上部消化管内視鏡検査: 胃液の吸引採取と胃腫瘍や胃炎の評価
胃疾患(腫瘍)の内視鏡施行医評価
他の胃疾患の有無 (胃炎, びらん, 萎縮, 消化性潰瘍など)は内視鏡施行医評価
内視鏡での胃炎重症度判定(胃炎の京都分類)
内視鏡検査画像は症例登録期間終了後に大分大学(事務局)に送付する
(4) H. pylori感染診断:
1. カルテより抽出(各施設)
便中抗原検査, 尿素呼気テスト, 病理組織学的検査で陰性
内視鏡検査や病理組織学的検査で胃粘膜萎縮や腸上皮化生、炎症細胞浸潤などを認めない
2. 胃液より評価(研究で採取した胃液よりPCRで感染診断を行う)
(5)胃液を使用したreal-time PCR法でNHPHとH. pyloriの感染診断を行う
胃液検体は研究参加施設で-20度以下にて冷凍保存をし、症例登録期間終了後に大分大学とミズホメディーに送付する
(6)解析: NHPH感染率,感染診断法(real-time PCR)の評価, 胃内疾患とNHPH感染との関連性, NHPHの内視鏡像、内視鏡施行原因疾患別のNHPH感染率

2) Degree of invasiveness: Minimally invasive (gastric fluid aspiration)
3) Study design: Multicenter, prospective, non-interventional
4) Participants: Patients with gastric tumors who were H. pylori-negative, or patients who had undergone treatment for gastric tumors, who visited the outpatient department of Oita University Hospital or a collaborative research facility for this study between the date of approval by the Dean of Oita University School of Medicine and March 31, 2027.
5) Specimen Collection Facilities (14 facilities)
6) Number of Subject Cases: 150 cases
7) Methods
(1) Obtaining informed consent
(2) Clinical information gathering: General medical history interview and abdominal symptom questionnaire
(3) Upper GI endoscopy: Gastric fluid aspiration sampling and evaluation of gastric tumors and gastritis
Endoscopist assessment of gastric disease (tumors)
Presence of other gastric diseases (gastritis, erosions, atrophy, peptic ulcers, etc.) evaluated by endoscopist
Endoscopic assessment of gastritis severity (Kyoto classification of gastritis)
Endoscopic images to be sent to Oita University (Administrative Office) after case registration period ends
(4) H. pylori infection diagnosis:
1. Extracted from medical records (each facility)
Fecal antigen test, urea breath test, Pathological histological examination: Negative
No gastric mucosal atrophy, intestinal metaplasia, or inflammatory cell infiltration observed by endoscopy or pathological histological examination
2. Evaluation from gastric fluid (Infection diagnosis performed by PCR using gastric fluid collected for the study)
(5) Infection diagnosis of NHPH and H. pylori using real-time PCR with gastric fluid
Gastric fluid specimens are cryopreserved at -20 degree or below at participating research facilities and shipped to Oita University and Mizuho Medical after the case registration period ends
