目的/Objectives

目的1/Narrative objectives1

日本語
咽喉頭異常感症症状から、加齢変化に伴うAb-ICAを早期発見し、脳梗塞を予防する診断法の提案。
提案することで、内頸動脈変異走行異常の患者が訴える唯一の症状である咽喉頭異常感症の診断精度の向上により、疾患概念の普及となり、長寿社会日本での国民の健康長寿増進のみならず、脳梗塞の発症の抑制効果による医療費大幅削減。

Proposal for a diagnostic method to detect abnormal internal carotid artery changes associated with aging early on, based on pharyngeal dysesthesia symptoms, thereby preventing cerebral infarction. This proposal will enhance the diagnostic accuracy of pharyngeal dysesthesia-the sole symptom reported by patients with abnormal internal carotid artery course variations-promoting understanding of the disease concept. This will not only advance public health and longevity in Japan's aging society but also significantly reduce healthcare costs by suppressing cerebral infarction incidence.

(1) 咽喉頭の異常感症状を訴え、原因精査目的で来院した患者（いわゆる咽喉頭異常感症）
(2) 満65歳以上の患者

(1) Patients presenting with pharyngeal dysesthesia symptoms and visiting the hospital for cause investigation (so-called pharyngeal dysesthesia syndrome)
(2) Patients aged 65 years or older

日本語
座位での、耳鼻咽喉科咽喉頭診察、内頸動脈変位走行異常(Ab-ICA)の有無を確認。
登録後スクリーニング診察時、Ab-ICAを認めた症例のみ、症例登録後6か月を観察期間とし、観察研究を行う。
Ab-ICA症例以外、観察は終了、最終診断名がついた時点で、各施設研究責任者が病名を確認、症例登録書に記載、登録終了とする。

Confirm the presence or absence of abnormal displacement of the internal carotid artery (Ab-ICA) during otolaryngological laryngopharyngeal examination in the seated position.
Only cases with Ab-ICA identified during the screening examination after registration will undergo an observational study with a 6-month observation period following case registration.
For cases without Ab-ICA, observation ends upon confirmation of the final diagnosis. The principal investigator at each facility will verify the diagnosis, record it on the case registration form, and complete registration.
