臨床画像所見（CT/MRI）、血清脂質・アディポカイン指標、探索的遺伝子解析を統合し、OPLL進展の病態メカニズムを多角的に評価する。

To integrate radiologic findings (CT/MRI), serum lipid/adipokine markers, and exploratory genomic analyses to comprehensively evaluate the pathophysiological mechanisms underlying OPLL progression.

1. 手術加療を予定している20～90歳の胸椎または頚椎OPLL患者
2. 対照群として手術を予定している15～40歳の非OPLL患者
3. 本人または代諾者から文書による同意を得た者

Patients aged 20to90 years with thoracic or cervical OPLL scheduled for surgery
Control group: nonOPLL patients aged 15to40 years scheduled for spinal surgery
Written informed consent obtained from the patient or legally authorized representative

本研究は観察研究である。
曝露（Exposure）：脂質代謝異常（内臓脂肪面積、酸化LDL、脂質関連分子の局所発現など）
結果（Outcome）：靱帯骨化の進展度、靱帯内脂質・蛋白質動態の変化、画像所見（CT/MRI）、血清マーカー、組織学的評価、探索的遺伝子解析
前向き/後ろ向き：前向き（prospective）
サンプリング方法：多施設における連続症例登録、群間比較（OPLL群と非OPLL群、ケースコントロール解析を含む）

This is an observational study.
Exposure: Metabolic abnormalities such as visceral adiposity, oxidized LDL, and local expression of lipid-related molecules
Outcome: Progression of ligament ossification, changes in lipid/protein dynamics within ligament tissue, imaging findings (CT/MRI), serum markers, histological evaluation, and exploratory genomic analyses
Prospective/Retrospective: Prospective
Sampling method: Consecutive case enrollment at multiple institutions, with group comparisons (OPLL vs non-OPLL, including case-control analysis)
