（２）本調査の内容を理解することができる患者
（３）スマートフォンを使用しアプリが使用できる患者

(2) Patients who can understand the content of this survey
(3) Patients who use smartphones and can use applications

（１） 主治医が調査への参加が不可能と判断した患者
（２） 精神疾患のある患者
