目的/Objectives

目的1/Narrative objectives1

日本語
本研究は、人工膝関節全置換術（TKA）後の患者を対象に、荷重位膝関節屈曲動作における運動学的データと運動恐怖との関連を明らかにすることを目的とし、臨床介入方針の策定に資する科学的知見を得ることを目指す。さらに、運動恐怖を有する患者に対してリハビリテーション介入を実施し、運動恐怖および運動学的指標の改善効果も検証する。

This study aims to clarify the relationship between kinematic data and movement fear in weight-bearing knee flexion movements in patients who have undergone total knee arthroplasty (TKA), with the goal of obtaining scientific knowledge that will contribute to the establishment of clinical intervention guidelines. Furthermore, rehabilitation interventions will be implemented for patients with movement fear, and the effects of these interventions on movement fear and kinematic indicators will also be verified.

主要アウトカム評価項目/Primary outcomes

・術後1か月・3か月・6か月時点における運動学的データ（最大角度・最大角速度・切り返し時間・開始遅延）と課題に対する恐怖（FearVAS、TSK-11）の編相関係数
・ベースラインを術後1ヵ月とし、術後3ヵ月と術後6ヵ月における運動恐怖（FearVASとTSK-11）とPainVASの変化。

Correlation coefficients between kinematic data (maximum angle, maximum angular velocity, turnaround time, and start delay) and fear of movement (FearVAS and TSK-11) at 1, 3, and 6 months postoperatively. Baseline is 1 month postoperatively; changes in FearVAS, TSK-11, and PainVAS will be assessed at 3 and 6 months postoperatively.

医療法人寿山会喜馬病院において、同一医師により同一機種の人工膝関節全置換術を受けた患者を対象とする。

Patients who have undergone total knee arthroplasty (TKA) with the same implant model performed by the same surgeon at Toshiyama-kai Kiba Hospital.

除外基準/Key exclusion criteria

日本語
1.重度の認知機能低下により課題理解が困難な者，2.中枢神経疾患の既往のある者，3.過去3か月以内の他部位の手術歴がある者，4.他の下肢疾患（変形性股関節症，リウマチなど）を有する者，5.術前から慢性痛を有する者

1. Patients with difficulty understanding tasks due to severe cognitive decline, 2. Patients with a history of central nervous system disease, 3. Patients with a history of surgery on other parts of the body within the past 3 months, 4. Patients with other lower limb diseases (hip osteoarthritis, rheumatism, etc.), 5. Patients with chronic pain from before surgery.
