日本語
本研究の目的は、腹膜透析療法を受ける患者の在宅療養支援体制を構築するために、栃木県日光市において基幹病院と訪問看護師の連携強化プログラムを実装し、その実装アウトカムを評価することである。評価には、ORICスコアをはじめ、受容性・実行可能性・適合性等の指標を用い、混合研究法により多面的に把握する。

The purpose of this study is to implement and evaluate a collaborative program between a core hospital and visiting nurses to support home-based care for patients undergoing peritoneal dialysis (PD) in Nikko City, Japan. This implementation study aims to assess outcomes such as organizational readiness for change (ORIC), acceptability, feasibility, and appropriateness using a mixed-methods approach.

1)栃木県日光市内に所在する訪問看護ステーションに勤務している看護師であること 。
2)研究期間中に PD 患者への訪問看護を担当する可能性があること

1) Nurses who are employed at a home-visit nursing station located in Nikko City, Tochigi Prefecture.
2) Nurses who may be involved in providing home-visit nursing care to peritoneal dialysis patients during the study period.
除外基準/Key exclusion criteria

研究期間中に長期の休職・転勤等が予定されており、継続的な関与が困難と判断される者
