1) 疾患名：2型糖尿病と診断されている患者
2) 年齢：同意取得時において年齢が65歳以上90歳未満の患者
3) 性別：不問
4) 日常生活が自立している（厚生労働省 要介護認定に係る法令による「要介護」の定義に該当しない）者
5) 生活習慣病に関する診療科（内科･糖尿病･内分泌代謝内科･総合内科･循環器内科など）の外来に通院、もしくは入院している患者
6) 文書による被験者もしくは代諾者の同意：患者本人の自由意思による文書同意が得られた患者、もしくは代諾者の同意が得られた患者

1) Diagnosis: Patients diagnosed with type 2 diabetes mellitus
2) Age: Patients aged >=65 and <90 years at the time of consent
3) Sex: No restriction
4) Daily living status: Patients who are independent in activities of daily living and do not meet the criteria for "requiring long-term care" as defined by the Ministry of Health, Labour and Welfare in Japan
5) Medical care setting: Patients receiving outpatient or inpatient care in departments related to lifestyle-related diseases (e.g., internal medicine, diabetology, endocrinology and metabolism, general internal medicine, cardiology)
6) Informed consent: Patients who have provided written informed consent of their own free will, or patients for whom written consent has been obtained from a legally authorized representative

除外基準/Key exclusion criteria

日本語
1) 1型糖尿病を有する者
2) 重度の腎機能障害（クレアチニンクリアランス15 ml/min未満）を有する者
3) 重篤な肝機能障害（Child分類C）を有する者
4) 急性合併症、悪性腫瘍や重篤な疾患を有する者
5) 立位の姿勢が保てない者
6) 厚生労働省 要介護認定に係る法令による「要介護」の定義に該当する者
7) 過去6か月以内にステロイド治療、悪性腫瘍に対する治療を受けた者
8) 研究責任者、研究分担者が本研究の参加について適切でないと判断した患者

1) Patients with type 1 diabetes mellitus
2) Patients with severe renal impairment (creatinine clearance <15 mL/min)
3) Patients with severe hepatic dysfunction (Child-Pugh class C)
4) Patients with acute complications, malignancies, or other serious illnesses
5) Patients who are unable to maintain a standing posture
6) Patients who meet the criteria for "requiring long-term care" as defined by the Ministry of Health, Labour and Welfare of Japan
7) Patients who have received steroid therapy or treatment for malignancy within the past 6 months
8) Patients deemed inappropriate for participation in this study by the principal investigator or co-investigators

対象者の人権擁護：
本研究は「人を対象とする生命科学・医学系研究に関する倫理指針」、「世界医師会ヘルシンキ宣言」、「個人情報の保護に関する法律に基づく倫理原則」を遵守して実施する。研究対象者のプライバシーを保護するため、氏名、現住所の詳細、電話番号、Eメールアドレス、勤務先情報を取得しない。研究の結果が公表される場合にも研究対象者の身元のプライバシー保護に配慮する。

Study Design: Prospective observational study

Study Population: Older adults with type 2 diabetes mellitus

Participant Enrollment Period: From the date of approval by the Ethics and Conflict of Interest Committee of the National Center for Geriatrics and Gerontology (NCGG) to 9 months thereafter

Observation Period: 12 months from the enrollment of the final participant

Total Study Duration: From the date of approval by the Ethics and Conflict of Interest Committee of the NCGG until March 31, 2028

Planned Sample Size: 160 participants

Primary Outcome Measure: Physical function (Five-Times Sit-to-Stand Test)

Secondary Outcome Measures: Skeletal muscle mass, muscle strength, muscle quality
