日本語
敗血症患者は高頻度に敗血症性播種性血管内凝固（DIC）を合併する。血小板は本来、外因性微生物を局所に封じ込める免疫血栓の形成において重要な役割を果たすが、DICにおいては生理的凝固反応を逸脱した過凝固状態が生じる。過剰な凝固によって形成された微小血栓は全身の臓器還流を障害し、多臓器不全の原因となる。そのため、血小板はDIC患者の病態管理において極めて重要な因子であり、診断および治療反応性の指標としても活用されている。感染症をはじめとする炎症性疾患においては、血小板の消費および産生亢進により、平均血小板容積（mean platelet volume: MPV）や血小板分布幅（platelet distribution width: PDW）などの形態学的指標が変化することが知られている。我々は、これらの血小板形態変化が敗血症性DIC患者の予後や治療効果の判定に有用であると仮定し、その関連性を検証する後ろ向き観察研究を実施する。

Patients with sepsis frequently develop sepsis-induced disseminated intravascular coagulation (DIC). Platelets play an essential role in forming immunothrombi that localize exogenous pathogens. However, in DIC, the coagulation process becomes dysregulated, resulting in a pathological hypercoagulable state. Excessive microthrombus formation impairs systemic organ perfusion and contributes to multiple organ dysfunction. Therefore, platelets are critical for the management of DIC and are widely utilized as indicators for both diagnosis and therapeutic monitoring. Inflammatory conditions such as infection lead to increased platelet consumption and production, causing significant alterations in morphological indices, including mean platelet volume (MPV) and platelet distribution width (PDW). We hypothesize that these morphological changes are associated with prognosis and treatment response in patients with sepsis-induced DIC. To test this hypothesis, we conducted a retrospective observational study.

本研究は単施設で実施される後ろ向き観察研究であり、2020年1月から2024年4月までに当院集中治療室に入室し、敗血症性DICと診断された患者を対象とする。診療録より血小板形態指標（MPV、PDW）を抽出し、死亡率との関連を検討する。

英語
This is a single-center, retrospective observational study. We will include patients admitted to the ICU with a diagnosis of sepsis-induced DIC between January 2020 and April 2024. Platelet morphological indices (MPV, PDW) will be extracted from medical records and analyzed in relation to clinical outcomes such as mortality.
