YDSとE-PASSを組み合わせて高齢者の術後譫妄発症のリスク因子を多角的に評価し、さらに術前の認知機能障害・術後譫妄発症が消化器癌術後の患者の予後に与える影響を明らかにすることを目的としている。

The aim of this study is to comprehensively evaluate the risk factors for postoperative delirium in elderly patients by combining the Yamaguchi Mental Disorder Scale (YDS) and E-PASS, and to clarify the impact of preoperative cognitive impairment and the development of postoperative delirium on the prognosis of patients undergoing surgery for gastroenterological cancer.

選択基準/Key inclusion criteria

1.手術時年齢が80歳以上の患者
2.消化器癌（食道癌、胃癌、結腸・直腸癌、肝癌、胆道癌、膵癌のいづれか）に対する予定手術を受けた患者
3.術前にYDS評価を受けた患者
4.術後譫妄の有無が記録されている患者

1.Patients aged 80 years or older at the time of surgery
2.Patients who underwent elective surgery for gastrointestinal cancer (esophageal, gastric, colorectal, liver, biliary tract, or pancreatic cancer)
3.Patients who underwent preoperative evaluation using the Yamaguchi Mental Disorder Scale (YDS)
4.Patients with documented presence or absence of postoperative delirium

除外基準/Key exclusion criteria

1.術前から意思疎通が困難な患者（JCS10～300点）
2.術前より明らかな譫妄を認める症例
3.重複癌

術後譫妄のリスクを軽減するためには、そのリスク因子を術前に適切に評価し、予防策を講じることが必要である。しかし、従来の認知機能評価方法である長谷川式簡易知能評価スケール（HDS)は、外科病棟での使用にはやや煩雑で、迅速なリスク評価が難しいという課題があった。この問題を解決するために、当科ではYamaguchi Mental Disorder Scale (YDS)を独自に開発し、これを用いた術前の認知機能評価により、譫妄リスクが高い患者を特定する試みを行ってきた。これまでの研究で、YDSスコアが低い患者は術後譫妄のリスクが高いことが示されている。また、手術リスク評価法であるE-PASS（Estimation of Physiologic Ability and Surgical Stress）は、手術侵襲や患者の生理機能の観点から術後のリスクを評価するためのものであり、術前および術中のリスク要因の統合的な評価を可能にすると同時に、譫妄発症リスクにも関与が示唆されている。

To reduce the risk of postoperative delirium, it is essential to appropriately assess risk factors preoperatively and implement preventive measures. However, the conventional cognitive assessment tool, the Hasegawa Dementia Scale (HDS), has been somewhat cumbersome for use in surgical wards, making rapid risk evaluation challenging. To address this issue, our department independently developed the Yamaguchi Mental Disorder Scale (YDS). Using this tool for preoperative cognitive assessment, we have attempted to identify patients at high risk of developing delirium. A previous study has shown that patients with low YDS scores are at higher risk for postoperative delirium. In addition, the Estimation of Physiologic Ability and Surgical Stress (E-PASS), a surgical risk assessment method, evaluates postoperative risk based on surgical stress and the patient's physiological status. It enables comprehensive assessment of both preoperative and intraoperative risk factors, and has also been suggested to be associated with the risk of developing delirium.
