The results of the descriptive statistics showed that ward nurses' role expectations from cancer patients and their families were higher than their role expectations from non-cancer patients and their families. Furthermore, the results of the multiple logistic regression analysis indicated that ward nurses' role recognition regarding ACP was influenced by recognition of the importance of ACP in the ward and presence or absence of ACP educators.

13施設に調査を依頼し、10施設(76.9%)から協力を得た。対象施設における対象者1,380人に説明文書を配布し、148人(10.7%)から回答を得た。小児科に勤務する2名を除外し、146人(98.6%)を分析対象とした。

We requested cooperation from 13 facilities and received cooperation from 10 facilities. We distributed explanatory documents to 1,380 subjects at the target facilities and received responses from 148 subjects. Excluding two subjects working in pediatrics, 146 subjects were included in the analysis.
参加者の流れ/Participant flow

各施設の看護部長に書面にて調査依頼を行い、承諾を得られた施設において、看護部を通し対象者に説明文書を配布した。対象者には説明文書に記載されている2次元バーコードにアクセスし、回答することを依頼した。調査票への回答期間は1ヶ月とした。

We sent written requests for participation in the survey to the nursing directors of each facility, and once we got the go-ahead, we gave the info sheet to the people involved through the nursing department. We asked them to scan the QR code on the info sheet and fill out the survey. The survey was open for one month.

1. 対象者背景
性別、年齢、臨床経験年数、病院機能、施設内のACP教育者の有無、ACP勉強会に参加した回数、緩和ケア・ホスピスに従事した年数、診療科を設定した。また、先行研究を参考に、質問項目として「ACPを開始するのは誰であることが望ましいと感じるか」を追加した。
2. ACPに対する病棟看護師の役割認識
田代らが作成した、がん患者に対するACPの看護支援尺度における構成要素を参考に、「ACP教育を行う」の1項目を追加し計51項目で測定した。本研究の病棟看護師の役割認識の定義に従い、「患者やその家族」「他の医療職種」から実践することを期待されている役割の認識を調査した。また、病棟看護師の役割認識ががん患者と非がん患者により異なる可能性があることから、患者やその家族については「がん患者・家族」「非がん患者・家族」それぞれについて調査した。回答者が「がん患者・家族」「非がん患者・家族」「他の医療従事者」から、ACPを実践する役割が期待されていると感じるかを2件法で尋ねた。
3. ACPに対する重要性の認識
Nakazawa K, et al.が作成したACPの重要性の認識に関する尺度を使用した。ACPは組織的な取り組みが重要であり、病棟看護師のACPにおける役割認識には、所属する病棟におけるACPの重要性が関連している可能性がある。本研究においては、「対象のACPに対する重要性の認識」と同時に、「病棟におけるACPの重要性」もACPの重要性の認識であるものとし、15項目について5段階リッカートスケールでそれぞれ測定した。
4. ACPの困難度
Nakazawa Y, et al.が作成した「緩和ケアに対する困難度尺度」を一部変更して使用した。さらに、看護師のACP実践の困難感として明らかになっている時間的要因について、Mori M, et al.が作成した「終末期に関する信念尺度」を参考に1項目を追加し、計15項目について5段階リッカートスケールで測定した。

1. Participant Background
Gender, age, years of clinical experience, hospital function, presence of ACP educators within the facility, number of times participating in ACP study sessions, years of experience in palliative care/hospice, and department were set. Additionally, referencing previous studies, the question item "Who do you feel should initiate ACP?" was added.
2. Ward Nurses' Perception of Their Role in ACP
Referencing the components of the nursing support scale for ACP in cancer patients developed by Tadashi et al., we added one item, "Conducting ACP education" and measured a total of 51 items. Following the definition of role perception for ward nurses in this study, we investigated the perceived roles expected to be practiced by "patients and their families" and "other healthcare professionals". Additionally, since ward nurses' role perceptions may differ between cancer patients and non-cancer patients, we surveyed "cancer patients and their families" and "non-cancer patients and their families" separately. Respondents were asked to indicate whether they felt that roles in practicing ACP were expected from "cancer patients and their families", "non-cancer patients and their families" and "other healthcare professionals" using a two-point scale.
3. Perception of the Importance of ACP
The scale developed by Nakazawa K, et al. was used to measure the recognition of the importance of ACP. ACP requires organizational efforts, and the recognition of ward nurses' roles in ACP may be related to the importance of ACP in their respective wards. In this study, both "recognition of the importance of ACP for the subject" and "importance of ACP in the ward" were considered as recognition of the importance of ACP, and each was measured using a 5-point Likert scale for 15 items.
4. Difficulty of ACP
We modified and used the "Palliative Care Difficulty Scale" developed by Nakazawa Y, et al. Additionally, regarding the time-related factors that have been identified as sources of difficulty in nurses' ACP practices, we added one item based on the "Beliefs About End-of-Life Scale" developed by Mori M, et al., and measured a total of 15 items using a 5-point Likert scale.

病棟看護師は患者の意思決定の過程におけるACP実践に対して重要な役割を持つ。ACPを実践する役割があるという看護師の認識はACP実践に影響を与える可能性があるが、その実態や関連要因は明らかになっていない。本研究の目的は、病棟看護師を対象とし、ACPに対する役割認識の実態とその関連要因を明らかにすることである。A県内10施設において、病棟看護師を対象に質問票調査を行った。記述統計の結果、病棟看護師の「がん患者・家族からの役割期待」は「非がん患者・家族からの役割期待」よりも高かった。また、多重ロジスティック回帰分析の結果、病棟看護師のACPに対する役割認識には「病棟のACPの重要性の認識」(OR=2.21~2.214)と「ACP教育者の有無」(OR=4.297~7.170)が影響を与えていた。病棟看護師のACPに対する役割認識を高めるためには、病棟が組織の風習としてACPの重要性の認識を高める必要性、および各医療機関においてACP教育が可能な組織員の養成・配置をする必要性が示唆された。

Ward nurses play an important role in the process of ACP practice in patient decision-making. Nurses' recognition of their role in practicing ACP may influence ACP practice, but the actual situation and related factors are not yet clear. The purpose of this study was to investigate the actual state of nurses' role recognition regarding ACP and its associated factors among ward nurses. A questionnaire survey was conducted targeting ward nurses at 10 facilities within A Prefecture. Descriptive statistical results showed that ward nurses' role expectations from cancer patients and their families were higher than those from non-cancer patients and their families. Additionally, the results of multiple logistic regression analysis indicated that ward nurses' role perceptions regarding ACP were influenced by recognition of the importance of ACP in the ward and presence of ACP educators. To enhance ward nurses' role recognition regarding ACP, it was suggested that wards need to elevate awareness of the importance of ACP as an organizational norm, and that each healthcare institution should cultivate and deploy staff capable of providing ACP education.
