選択基準/Key inclusion criteria

日本語
急性期病棟に入院する高齢患者（70～105歳）を対象に適格性の評価を行い、1．せん妄ハイリスク患者ケア加算の要件に基づく【資料2 せん妄リスク評価】でリスク要因が「有」と判定された者、2．精神科リエゾンチームへの相談内容が「せん妄ケア」「認知症」に該当する者を選定する。

Elderly patients (70-105 years old) admitted to acute care wards will be evaluated for eligibility, and 1. those whose risk factors are determined to be Yes by 2 Delirium Risk Assessment based on the requirements for the additional care for high-risk patients with delirium, and 2. those whose consultation with the psychiatric liaison team falls under delirium care or dementia. Select patients with delirium care and dementia.

除外基準/Key exclusion criteria

日本語
1.言語的に意思疎通が困難な者(重度の認知症,感覚性失語症のある者).
2.病状が重篤,重症な者.
3.終末期状態の者.
4.新型コロナウイルス,インフルエンザ等の感染症に罹患もしくは罹患の疑いのある者.
5.主治医または病棟師長が,ボランティアの介入が「不可」と判断した者.

1. patients with difficulties in verbal communication (severe dementia or sensory aphasia).
2. patients with serious illness or critical illness.
3. terminally ill patients.
4. patients who have or are suspected to have an infectious disease such as new coronavirus or influenza.
5. patients who are deemed unfit for volunteer intervention by the attending physician or ward manager.
