頸部前庭誘発筋電位（cVEMP）は、聴覚刺激に対して同側の胸鎖乳突筋に記録される筋反応です。これは聴覚刺激によって引き起こされますが、前庭系の耳石器官である球形嚢に由来する特異な反応です。私たちはこれまでにcVEMPの周波数特性について研究し、その臨床応用について報告してきました。近年、振幅変調音刺激を用いたcVEMP（AMcVEMP）が報告されており、その臨床応用が期待されています。本研究では、当施設においてAMcVEMPの記録を試み、その臨床的意義について検討しました。

Cervical vestibular evoked myogenic potential (cVEMP) is a myogenic response recorded in the ipsilateral sternocleidomastoid muscle in response to auditory stimulation. Although it is a response to auditory stimulation, it is a unique reaction originating from the saccule of the vestibular system. We have previously studied the frequency response of cVEMP and reported on its clinical applications. Recently, cVEMP using amplitude-modulated (AM) sound stimulation (AMcVEMP) has been reported, and its clinical application is expected. In this study, we attempted to record AMcVEMP at our facility and considered the significance of AMcVEMP.

被験者に対し、説明書をもとに本試験について説明し、同意を取得する。
周波数計信号源モジュールを用いて変調波を作成する。搬送波を500Hz、変調波を17Hz, 23Hz, 37Hz, 113Hz, 211Hzとし、100％変調する。音圧は95dB LAとする。挿耳型イアホンより両耳に与える。
既存のcVEMPの測定と同様、探査電極は胸骨頭付着部と乳様突起の中間点に、基準電極は胸鎖乳突筋の胸骨頭付着部に、接地電極は前額部に貼付する。胸鎖乳突筋の筋緊張を確保するために被検者は仰臥位で音刺激と反対側に頸部を捻転させる。筋電図はNeuro Packを用いて測定する。バンドパスフィルターは20Hz～2kHzとする。持続時間1secの刺激音における反応を、100回加算平均する。音を与えない場合の筋電図と一般的なcVEMP、搬送波のみを聴かせた時の筋電図も測定する。所要時間は1時間程度である。

The subjects will be provided with an explanation of the study based on the instruction manual, and their consent will be obtained. The modulated wave will be generated using a frequency counter signal source module. The carrier wave will be set at 500 Hz, and the modulated waves will be set at 17 Hz, 23 Hz, 37 Hz, 113 Hz, and 211 Hz, with 100% modulation. The sound pressure level will be set to 95 dB LA. The sound will be delivered to both ears via insert earphones.
As with the existing cVEMP measurements, the reference electrode will be placed at the mid-point between the sternal head attachment of the sternocleidomastoid muscle and the mastoid process, the ground electrode will be placed on the forehead, and the exploration electrode will be placed at the sternal head attachment of the sternocleidomastoid muscle. To ensure muscle tension in the sternocleidomastoid muscle, the subject will be positioned supine, with their neck rotated away from the side of the sound stimulation. Electromyography will be measured using the Neuro Pack system. The bandpass filter will be set to 20 Hz to 2 kHz. Responses to 1-second stimulation tones will be averaged over 100 trials. Electromyography will also be measured under conditions with no sound stimulation, typical cVEMP, and with only the carrier wave. The total duration of the procedure will be approximately one hour.

1)これまでめまい、難聴など神経耳科学的疾患の既往のないもの。
2)14時から18時に試験実施場所に来訪可能なもの。
3)年齢は同意取得時において年齢が20歳以上65歳未満の男女。
4)本試験の参加にあたり十分な説明を受けた後、十分な理解の上、被験者本人の自由意思による文書同意が得られたもの。

1)Individuals with no prior history of neuro-otological disorders such as vertigo or hearing loss.
2)Individuals who are able to visit the testing site between 2:00 PM and 6:00 PM.
3)Male and female participants aged 20 years or older and under 65 years at the time of consent.
4)Individuals who have received a full explanation of the study, understood it sufficiently, and provided written informed consent of their own free will.
除外基準/Key exclusion criteria

以下のいずれかに抵触するものは本研究に組み入れないこととする。
1) 開始1週間前に抗めまい薬、精神安定剤など検査結果に影響を及ぼす薬剤を使用したもの。
2) 船酔い、自動車酔いなど動揺刺激で気分不良・悪心の生じやすいもの。
3) 妊娠中あるいは妊娠の可能性がある女性。
4) 頸動脈狭窄症や頚椎症等、頸部捻転で増悪する可能性がある頸部疾患の既往のあるもの。
5) 授乳中の女性。
6) その他、研究責任(分担)者が被験者として不適当と判断したもの。

Individuals meeting any of the following criteria will be excluded from participation in this study:
1)Individuals who have used medications that may affect the test results, such as antivertigo drugs or tranquilizers, within one week prior to the start of the study.
2)Individuals who are prone to motion sickness, such as seasickness or carsickness.
3)Women who are pregnant or may possibly be pregnant.
4)Individuals with a history of cervical conditions that may be aggravated by neck rotation, such as carotid artery stenosis or cervical spondylosis.
5)Women who are currently breastfeeding.
6)Any other individuals deemed unsuitable as study subjects by the principal or sub-investigator.
