日本語
急性期の末梢性顔面神経麻痺（以下、FP: Facial palsy）患者を対象とし、鍼通電刺激によって誘発される表情筋収縮反応の程度の差異と顔面神経麻痺評価ツールである柳原法によりグループ分けを行い、グループの違いからFP患者に対する鍼治療の効果を推定することを目的とする。

The aim of this study is to estimate the therapeutic effects of acupuncture in patients with acute peripheral facial palsy (FP) by classifying them into groups based on differences in facial muscle contraction responses (FMCR) induced by electroacupuncture stimulation and the Yanagihara score, a commonly used facial palsy assessment tool.

モデル1：FMCRのみで定義されるグループによるモデル、モデル2：FMCRと柳原法により定義されるグルーブによるモデルにおける各グループ間の治癒基準到達までのハザード比と、発症から 30 日、
90 日、180 日、360 日時点の time-dependent AUC を算出し、改訂 DeLong 法で検定を行い、Holm の検定による多重比較を行う。

Two predictive models will be constructed in this study. Model 1 will classify patients into groups based solely on the facial muscle contraction response (FMCR), whereas Model 2 will classify patients into groups based on both the FMCR and the Yanagihara score. Within each model, hazard ratios for achieving the predefined recovery criteria will be estimated for each group. In addition, time-dependent area under the curve (AUC) values at 30, 90, 180, and 360 days after onset will be calculated and compared between the two models using the modified DeLong test, with Holm's procedure applied to adjust for multiple comparisons.

Differences in electroneurography (ENoG) values between Model 1 and Model 2 will also be examined.
In addition, hazard ratios for achieving the predefined recovery criteria will be estimated for comparisons among groups within each model: Model 1, Model 2, and Model 3 will classify patients into groups based on both electroneurography (ENoG) values and the Yanagihara score. Model 4 will classify patients into groups based on the Yanagihara score. Model 5 will classify patients into groups based on ENoG values alone. Hazard ratios for achieving the predefined recovery criteria will be estimated for comparisons among groups within each model. In addition, time-dependent AUC values at 30, 90, 180, and 360 days after onset will be calculated and compared using the modified DeLong test, with Holm's procedure applied to adjust for multiple comparisons

選択基準/Key inclusion criteria

1）2016年6月1日から2023年12月31日に埼玉医科大学病院東洋医学診療科で鍼治療を受療した末梢性顔面神経麻痺患者（鍼治療は患側顔面神経近傍への鍼通電療法や表情筋への刺鍼など）
2）発症2週間以内の患者
3）ENoG一般法を実施した患者
4）FMCRの結果が電子カルテ上に記録されている患者
5) 発症2週間以内の柳原法の結果が電子カルテ上に記録されている患者

1.Patients with peripheral facial palsy who received acupuncture treatment at the Department of Oriental Medicine, Saitama Medical University Hospital between June 1, 2016, and December 31, 2023.
 (Acupuncture treatments included electroacupuncture near the affected facial nerve and needling of the facial muscles.)
2.Patients within two weeks of onset.
3.Patients who underwent ENoG using the modified NLF method.
4.Patients whose FMCR results were recorded in the electronic medical record.
5.Patients whose Yanagihara scores within two weeks of onset were recorded in the electronic medical record.

除外基準/Key exclusion criteria

日本語
1）中枢性および末梢性顔面神経麻痺の既往がある者
2）発症日、既往歴、投薬歴が不明であるもの
3）末梢性顔面神経麻痺の原因について診断がついていないもの（例：Bell麻痺と診断するに十分な鑑別が行われていないもの）
4）ENoG値が100％以上である者
5）ENoGが発症から2週間以降に実施されたもの
6）本研究の対象から除外を希望する申し出があった者

1.Patients with a history of either central or peripheral facial nerve palsy.
2.Patients with unknown onset date, medical history, or medication history.
3.Patients without a confirmed diagnosis of the cause of peripheral facial palsy (e.g., insufficient differential diagnosis to conclude Bell's palsy).
4.Patients with ENoG values equal to or greater than 100%.
5.Patients who underwent ENoG more than two weeks after onset.
6.Patients who expressed a desire to be excluded from the study.
