本研究の目的は、在宅で介護が必要な高齢者を対象に、多職種チームによる個別の食事支援計画を作成し、それに基づいた食事支援の有効性を検証することである。

（1）MNA-SF（雨海，2011）による評価点が8点以上11点以下の「低栄養のおそれがあり」または7点以下の「低栄養」の状態である者とした．
（2）経口摂取可能

(1) Subjects were defined as those who were at risk of malnutrition, with an assessment score of 8 to 11 on the MNA-SF (Ameumi, 2011), or those who were in a state of malnutrition, with a score of 7 or less.
(2)Ingestible

除外基準/Key exclusion criteria

（2）経静脈栄養を利用している者
（3）調査により心理的負担を生じる可能性がある者
