目的/Objectives

日本語
この研究の目的は、変形性膝関節症に対する体外衝撃波治療(ESWT)の治療効果、PRP療法やAPS療法、MSC療法といった再生医療の治療効果、そしてESWTとそれら再生医療との併用療法の治療効果を、Ｎクリニックで行った患者のデータをもとに後ろ向き調査により検証することである。

The purpose of this study is to verify the therapeutic efficacy of extracorporeal shock wave therapy (ESWT) for knee osteoarthritis, the therapeutic effect of regenerative medicine such as PRP therapy, APS therapy, and MSC therapy, and the therapeutic efficacy of ESWT and their combination therapy with regenerative medicine by retrospective survey based on patient data conducted at N Clinic.

Ｎクリニックに来院した患者の中で、Nクリニック或いは他院でX線撮影で変形性膝関節症の診断を受けヒアルロン酸の関節内注射を受けるも、痛みが改善しない症例

Patients who were diagnosed with knee osteoarthritis by X-ray at N Clinic or another hospital, and received intra-articular injections of hyaluronic acid, but the pain did not improve

除外基準/Key exclusion criteria

日本語
すでに変形性膝関節症に対する手術や他の治療を受けて、その治療効果が関与していると思われる患者、RAなどの代謝性疾患や感染性疾患を有する患者は研究対象からすべて除外する

Patients who have already undergone surgery or other treatments for knee osteoarthritis and those therapeutic effects are likely to be involved, as well as patients with metabolic or infectious diseases such as RA, are excluded from the study.

Nクリニックでは変形性膝関節症の痛みの原因の一つであるBML由来の骨の痛みに対して集束型の体外衝撃波治療（ESWT）を行ってきて優れた効果を示してきたが、効果は限定的であった。また関節腔内の痛みの原因である滑膜炎に対して2018年からPRP（platelet-rich plasma)療法やAPS(Autologous protein solution)療法の関節内注入を行ってきたが、これも効果が限定的であった。そこで滑膜炎とBMLの両方を有する症例に対してAPS療法とESWTの組み合わせ療法を行ってきてさらに優れた効果を示してきた。しかしながら、ESWTだけでもAPS療法とESWTの組み合わせ療法でも治療効果が限定的であった。治療効果が芳しくない症例には、骨壊死や骨嚢胞を伴うBMLや軟骨下骨プレートの断裂を伴ういわゆる難治性BMLというものが存在していることが分かった。そこで軟骨下骨プレートを改善させる目的でそれら難治性BMLを有する膝ＯＡの症例に対して、SC（mesenchymal stromal cell）を関節内と骨内に注入し、更にESWTをMSCの注入前と注入後に行っていくといったMSCとESWTの組み合わせ療法を開始した。

At N clinic, focused extracorporeal shockwave therapy (ESWT)has been performed on bone pain caused by BML, which is one of the causes of knee osteoarthritis pain, and has shown excellent effects, but the effect has been limited. In addition, intra-articular injections of platelet-rich plasma (PRP) therapy and autologous protein solution (APS) therapy have been performed for synovitis, which is the cause of pain in the joint cavity, but the effect of these therapy has also been limited. Therefore, a combination of APS therapy and ESWT therapy has been performed in patients with both synovitis and BML, and has shown even better effects. However ESWT alone and the combination of APS therapy and ESWT had a limited therapeutic effect.
It was found that there are so-called refractory BML with osteonecrosis and bone cysts and so-called refractory BML with rupture of the subchondral bone plate in cases where the treatment effect is not good. Therefore, for the purpose of improving the subchondral bone plate, MSC(mesenchymal stromal cell) was injected intraarticular and intraosseous, and ESWT was performed before and after the injection of MSC for patients with knee OA with refractory BML.
