本研究はAMIのためPCIが施行された患者の、非責任病変における冠動脈残存プラークに対する脂質低下療法の有効性を検討するための探索的研究であることから、評価項目や解析方法に関しては、データ収集後に研究代表者や研究担当者が決定することとする

Since this is an exploratory study to determine the efficacy of lipid-lowering therapy for residual coronary plaque in non-culprit lesions in patients undergoing PCI for AMI, the principal investigator or investigators will determine the endpoints and analysis methods after data collection.

選択基準/Key inclusion criteria

下記の適格基準を全て満たす患者
① 18歳以上、85歳以下
② AMIに対してPCIを受けた際に以下のいずれかの残存病変を有する
1）冠動脈造影で25％以上75％未満の狭窄
2）冠動脈造影で75％以上90％未満の狭窄かつ、FFR>0.80もしくはResting Index >0.89
③ 入院中の空腹時LDL-C値が55 mg/dL以上
④ 研究参加に対して本人から文書による同意が得られる

Patients who meet all of the following eligibility criteria
(1) 18 years of age or older and less than 85 years of age
(2) Have one of the following residual lesions at the time of PCI for AMI
i) Coronary angiographic stenosis between 25% and 75%.
ii) Coronary angiographic stenosis of 75% to less than 90% and FFR >0.80 or Resting Index >0.89
(3) Fasting LDL-C level >55 mg/dL during hospitalization
(4) Written consent for participation in the study is obtained from the patient.

除外基準/Key exclusion criteria

① eGFR<30 ml/min/1.73m2
② PCSK9阻害薬もしくは持続型LDL-C低下siRNA製剤を使用中もしくは使用歴がある
③ 研究の対象となる冠動脈残存病変に対してPCIもしくはCABGが予定されている
④ 複雑先天性心疾患の合併
⑤ 恒久的ペースメーカー又は体内除細動リードを有する患者
⑥ 人工心臓弁置換後の患者
⑦ CTデータの取得を妨げる不整脈又は頻脈を有する患者
⑧ ボディマス指数（BMI）>35 の患者
⑨ 硝酸薬が禁忌の患者
⑩ 1 年以上の予後が見込めない患者
⑪ 他の薬剤による臨床試験に参加中の患者
⑫ 患者の安全もしくはプロトコル順守が妨げられると考えられる状況
⑬ 研究責任医師又は研究分担医師により不適と判断された患者

(2) Current or historical use of PCSK9 inhibitors or sustained LDL-C-lowering siRNA agents
(3) Scheduled PCI or CABG for residual coronary artery lesions under study
(4) Complicated congenital heart disease
(5) Patients with a permanent pacemaker or internal defibrillation leads
(6) Patients after artificial heart valve replacement
(7) Patients with arrhythmia or tachycardia that interferes with CT data acquisition
(8) Patients with body mass index (BMI) >35
(9) Patients with contraindication to nitrates
(10) Patients with no prognosis for more than 1-year
(11) Patients who participated in clinical trials with other drugs
(11) Patients participating in clinical trials with other drugs
(12) Patients under conditions that may interfere with patient safety or protocol compliance
(13) Patients who are deemed unsuitable by the principal investigator or sub investigator
