日本語
妊娠糖尿病（GDM）は、妊娠の経過によりインスリン抵抗性が増大し、血糖コントロールが不十分となった場合にインスリン治療が必要になる。インスリン抵抗性指標をもとに、インスリン治療を必要とするGDM発症妊婦をより効果的に予測できる因子について検討した。

The aim is to identify pregnant women with GDM who will require insulin administration as a prognosis, using an insulin resistance index in pregnant women with abnormal glucose metabolism.

主要アウトカム評価項目/Primary outcomes

インスリン抵抗性指標としてHOMA-IR、QUICKI、Matsuda indexを後方視的に解析した。GDM妊婦の治療経過の予測について、目的変数をGDMの有無、説明変数をそれぞれのインスリン抵抗性指標とした単変量ロジスティック回帰解析、ROC曲線のAUC解析を行った。

各指標の至適カットオフ値を用いたインスリン投与の有無の識別結果から、Matsuda Indexのカットオフ値3.74における、陽性尤度比1.99は最も高く、陰性尤度比0.40が最も低かった。

The AUCs for the ability of the HOMA-IR, QUICKI and Matsuda indices to identify GDM were 0.618, 0.618 and 0.744, respectively, with the Matsuda index being the most significant (p < 0.0001). The optimal cutoff values for the HOMA-IR, QUICKI, and Matsuda indices were 1.72, 0.352, and 3.74, respectively.
