群馬大学医学部附属病院成育医療センターに入院、または小児科外来に通院し、糸球体疾患、または、先天性腎尿路形態異常の診断を受けている5歳～12歳の小児

- Diagnosed with renal disease or congenital renal urinary system malformation.
- Admitted to the Child Health and Development Center or seen at the pediatric outpatient of The Gunma University Hospital
