本研究の目的は、膝の横ぶれ、床反力、足圧中心、関節モーメントなどの変数を含む歩行分析を用いて、術後早期の歩行機能の改善を検証することである。

整形外科医により変形性膝関節症の診断を受け、人工膝関節全置換術の予定がある症例。

Patients diagnosed with knee osteoarthritis by an orthopedic surgeon and scheduled for total knee arthroplasty.

除外基準/Key exclusion criteria

日本語
関節リウマチ、外傷による膝関節の手術歴、中枢神経障害。自力歩行が困難な患者も除外。

The exclusion criteria for this study were prior history of rheumatoid arthritis, surgery on the knee joint, trauma, or central neuropathy. Patients who had difficulty walking on their own were also excluded.

TKA後のアライメント矯正により、膝の横ぶれは改善した。横方向の床反力が減少したことは、横方向の剪断力による力学的負荷が軽減したことを示唆する。また、TKAにより膝関節の伸展機能が改善し、床反力の垂直成分が減少したことから、立脚中期における力学的負荷が軽減したことが示唆された。

The varus thrust was improved by the alignment correction after TKA. The reduction of the GRF in the lateral direction suggests that the mechanical load by the lateral shear force was reduced. In addition, the knee joint extension function improved and the vertical component of the GRF was reduced after TKA, indicating a reduction of the mechanical load in the mid-stance phase.
