初回急性心筋梗塞患者を対象に、新規に開発した測定系を用いてナトリウム利尿ペプチド系の構成分子を個別に測定し、各分子濃度あるいは比率の臨床的意義について心筋梗塞後リモデリング・予後を検証する。

In patients with first acute myocardial infarction, individual components of the natriuretic peptides will be measured using a novel developed assay, and the clinical significance of each molecule concentrations will be examined to clarify their role of post-myocardial infarction remodeling and prognosis.

近年の研究でANP、BNPの中には種々の個別分子の存在が明らかとなりつつある。本研究では初回発症の急性心筋梗塞症例を対象に血漿中ナトリウム利尿ペプチドの各種分画を計測するとともに、患者背景、薬物療法といった背景因子に急性心筋梗塞後急性期心筋障害程度およびその後の左室容量の変化・長期予後等との関連性を解析して、急性心筋梗塞後左室リモデリング発症、心血管イベントを推定する信頼度の高い診断技術を開発することを目的とする。

Recent studies have shown the clinical significance measuring various molecular forms of natriuretic peptides in patients with heart diseases. In this study, we will measure plasma various forms of natriuretic peptides including precursor and active forms of natriuretic peptide as well as their surrounding hormones and their downstream molecules with clinical findings in patients with first acute myocardial infarction. This integrated analysis may provide a reliable method for predicting left ventricular remodeling after acute myocardial infarction and future events based on measurements of individualized molecules in the natriuretic peptide system.

(1) 発症-再灌流時間　12時間以内　（２）初回急性心筋梗塞　（３）Killip分類１度　(4) 血清クレアチニン値<1.5mg/dl (4) ヘモグロビン値 11g/dl以上

(1) onset-to-reperfusion time within 12 hours, (2) first episode of acute myocardial infarction, (3) Killip Class 1, (4) serum creatinine levels below 1.5 mg/dl, and (6) hemoglobin levels of 11 g/dl or more

除外基準/Key exclusion criteria

（１）急性期再灌流療法未施行　ないしは　最終造影でTIMI分類 0-1で終了している症例　（２）急性期多臓器不全合併例　（３）急性心筋梗塞合併　心原性ショック　ないしは　機械的合併症発生例　（４）緊急冠動脈バイパス手術実施例　（５）活動性悪性腫瘍　ないしはその既往のある症例　（６）神経疾患　認知症などで　自己判断ができない症例

(1) no reperfusion or final thrombolysis in myocardial infarction grade of 0 or 1, (2) presence of multiple organ failure, (3) occurrence of mechanical complications of AMI or refractory cardiogenic shock, (4) undergoing emergent or urgent CABG as a reperfusion therapy, (5) history of any malignancy, (6) experiencing difficulty in self-judgment due to neurological disease, mental illness, or dementia.

初回急性心筋梗塞患者を対象に、左室リモデリングと様々な分子型ナトリウム利尿ペプチドとの臨床転帰との関連を検討する前向き観察研究。
左室リモデリングを評価するために、登録された患者は急性心筋梗塞後急性期（10日）と慢性期（10-12ヵ月）の左室容積と機能を評価するために心エコー図検査と心臓磁気共鳴検査を2回実施する。
またナトリウム利尿ペプチド系および関連する分子を評価するため　PPCI直後、PPCI12時間後、3日後、5日後、10日後、1ヵ月後、3ヵ月後、10ヵ月後という8つの時点で、血液サンプルを採取、ナトリウム利尿ペプチド系および関連分子の計測を行う。
臨床フォロー（イベント）、本人・家族から　医療記録、アンケートを用いて収集する。

A prospective observational study to examine the association of left ventricular remodeling and clinical outcomes with various molecular forms of natriuretic peptides in patients with first acute myocardial infarction.
To evaluate left ventricular remodeling, enrolled patients will undergo serial echocardiogram and cardiac magnetic resonance to evaluate left ventricular volume and function at acute (10 days) and chronic phase (10-12months) after acute myocardial infarction. Blood samples for assessing plasma concentrations of various molecular forms of natriuretic peptides and their second messengers were collected at eight time points: immediately after PPCI, 12 hours after PPCI, 3 days, 5 days, 10 days 1 month 3 months and 10 months after AMI.
Clinical follow-up data, based on a standard questionnaire, were obtained through correspondence with patients, relatives, or physicians, or from hospital records.
