介入開始から１週間以内にAid for Decision making in Occupation Choice2 (ADOC2) を使用し合意目標の設定を行い，生活行為向上マネジメントシートを作成する．その後，運動療法および心不全指導に加えて生活行為向上マネジメントシートに準じて介入を進める．
介入は入院中のみに実施．実施回数は入院日数によって異なる．

Within one week of starting the intervention, the Aid for Decision-making in Occupation Choice 2 (ADOC2) was used to set agreed-upon goals and create a daily living activity management sheet. Subsequently, the intervention was carried out in accordance with the daily living activity management sheet, in addition to exercise therapy and heart failure education.
The intervention was only conducted during hospitalization. The number of sessions varied depending on the length of hospitalization.

2. 重度の認知症，失語症，精神疾患などを理由に意思疎通が困難な者
