目的1/Narrative objectives1

日本語
ヘルシー・エイジング社会の推進へ向けて、効果的な自治体支援のあり方を明らかにし、その介入効果を評価をする。そのために、健康の社会的決定因子(Social determinates of health: SDH)を踏まえた地域包括ケアシステム構築の推進支援ツールであるJAGES-HEARTを活用して、データ活用と組織連携を促し、自治体職員や住民自らによって健康なまちづくりへ向けた地域組織化が行えるように伴走型支援を行う。その介入効果を、要介護認定を受けていない高齢者のFunctional Ability Indexを主評価項目としてクラスターランダム化比較試験にて評価する。同介入による研究参加自治体における自然環境や防災への意識等、保健医療分野を超えた地域課題への効果(co-benefit)、研究参加自治体における介護予防及び自治体高齢者保健担当職員のワークエンゲージメント、政策立案能力や多職種連携状況への効果の評価も行う。

To promote a healthy aging society, we clarify effective municipal support methods and evaluate their intervention effects.For this purpose, we use JAGES-HEART, a support tool designed for the development of a comprehensive community care system based on the social determinants of health (SDH).This tool facilitates data utilization and organizational collaboration, enabling local government staff and residents to organize community efforts towards healthier towns.We evaluate the intervention effects through a cluster-randomized controlled trial using the Functional Ability Index of older people who have not received long-term care certification as the primary outcome measure. We will also assess the effects of the intervention on broader community issues beyond health, such as environmental awareness and disaster preparedness, as well as its co-benefits in participating municipalities.Additionally, the study will evaluate the effects on the prevention of need for care, work engagement of municipal elderly care staff, policy-making capabilities, and the status of multidisciplinary collaboration within these municipalities.

・各自治体毎に担当の研究者を設定し、各自治体の状況にあわせた地域組織化を促す伴走型支援
・提供する地域診断データを積極的に活用した、地域課題および資源同定の支援
・組織間連携、多部署連携の支援
・介護予防施策の立案、実施の支援

-Assign a responsible researcher in each municipality and provide hands-on support tailored to the situation of each municipality to promote community organization.
-Support for identifying community issues and resources by actively utilizing the provided community diagnosis data
-Support for inter-organizational collaboration and multi-departmental collaboration
-Support for planning and implementing long-term care prevention measures

1) 2022年に実施したJAGESコホート研究へと参加している。
2) 研究参加時点で65歳以上である
3) 研究参加時点で介護保険要介護認定(「要支援1~2」及び「要介護1~5」)を受けていない

【高齢者対象評価（サンプリング調査自治体）】
1) 研究参加時点において65歳以上である
2) 研究参加時点では介護保険要介護認定(「要支援1~2」及び「要介護1~5」)を受けていない
3) 1)2)を満たすものを、自治体の小学校区から最低150人無作為抽出し、研究に組み入れる。尚、150人に満たない場合はその小学校区に在住の1)2)を満たす全員を研究に組み入れる。
4) 3)にてサンプリングされたものの内送付した質問紙へと回答した者

1) 自治体の地域包括ケア推進部署の職員

1)They are participating in the JAGES cohort study conducted in 2022.
2)They are 65 years or older at the time of participation in the study.
3)They are not certified as needing long-term care insurance (support levels 1-2 and care levels 1-5) at the time of participation in the study.

[Evaluation for older people (Sample Survey Municipalities)]
1)They are 65 years or older at the time of participation in the study.
2)They are not certified as needing long-term care insurance (support levels 1-2 and care levels 1-5) at the time of participation in the study.
3)Those who meet criteria 1) and 2) are randomly selected from each elementary school district within the municipality, with a minimum of 150 individuals. If fewer than 150 individuals meet these criteria in a district, all eligible individuals residing in that district are included in the study.
4)Among those sampled in 3), only those who responded to the distributed questionnaire are included.
