本プロジェクト研究の目的は、A病院版Continuity and coordination of careを基盤とした「助産師外来」における継続ケア促進プログラムを実装戦略とし、妊娠中の糖代謝異常に関するEvidence-basedの実践転移の実装と評価を行うことである。

To implement and evaluate an evidence-based continuity of midwifery care program at the antenatal clinic of Hospital A, based on the continuity and coordination of care framework, for promoting continuous care and translating evidence into practice regarding gestational hyperglycemia.

本プロジェクト研究は、A病院の助産師外来においてWHOのContinuity and coordination of careという継続ケアと統合・調整の枠組みを基盤とした助産師外来における継続ケア促進プログラムを実装することにより、糖代謝異常をもつ女性に対する良質な継続的支援の質改善を図る実装研究である。本プロジェクトでは、助産師外来担当助産師を対象として継続ケアプログラムを実施し、ケアガイドを基に糖代謝異常の女性に妊婦健診を提供する。

This project study is an implementation research aimed at improving the quality of continuous support for women with gestational hyperglycemia by implementing a continuity of midwifery care promotion program at the antenatal clinic of Hospital A, based on the World Health Organization's Continuity and Coordination of Care framework. In this project, a continuity of care program will be implemented for midwives assigned to the antenatal clinic, and they will provide antenatal care to women with gestational hyperglycemia following the care guidelines.
