この研究の目的は、医療社会事業専門員が、入院患者に対して、ACP介入を行う前後で、入院患者のACPエンゲージメントを測定し、介入効果を推定することである。

The purpose of this study is to measure ACP engagement of hospitalized patients before and after an inpatient ACP intervention by health social work professionals and to estimate the effect of the intervention.

２．65歳以上の入院患者
３．ACPについての対話ができる入院患者
４．ACPについて相談できる家族らがいる入院患者
５．ACPについて相談できる医療者がいる入院患者

1. Inpatients assigned to a health and social services specialist
2. inpatients over 65 years old
3. Inpatients with whom ACP can be discussed.
4. Inpatients who have family members who can discuss ACP with them.
5. Inpatients who have a health care provider who can discuss ACP with them.
除外基準/Key exclusion criteria
日本語
１．これまでACPを行っている入院患者
２. 医療社会事業専門員が、精神的に不安定であり、ACP介入や質問紙調査の実施が望ましくないと判断した入院患者
３．身体的な病状が落ち着いておらずACPを行うことが望ましくないと判断した入院患者
４．医療社会事業専門員の関わる期間が、12週間未満と予測される入院患者

1. inpatients who have previously been administered ACP
2. inpatients who are deemed mentally unstable and undesirable for ACP intervention or questionnaire survey by a health and social services specialist
3. inpatients whose physical condition is not stable and for whom ACP is not advisable
4. inpatients with an expected length of stay of less than 12 weeks for which the health and social services specialist is expected to be involved.
